Our new Patient Portal makes it easier than ever to access your medical records and manage your health.

With our Patient Portal, you can:
✔️Schedule appointments online
✔️Request medication refills online
✔️Receive reminders to your cell and email
✔️Send secure messages to your provider

Patients will automatically receive an email invitation with a PIN to register for the portal after their appointment, typically within 24 hours. If you wish to enroll before your appointment, you will need to contact the clinic to obtain a PIN.

Come join us for our Grand Opening celebration tomorrow from 10:30 - 2:00p! We will have festivities and a first look at the newest health center in Coos Bay, Three Rivers Health Center!

Come learn more about our new services, and enroll as a new patient! We have full Pharmacy services opening to both our Tribal and Non-Tribal community on Monday June 3rd, 2024.

Currently seeing only Tribal Members, but we will be opening to the public (non-Tribal members) beginning on July 1st, 2024.

Medical (Immediate and Routine Care)
Behavioral Health (Psychiatry and Therapy)
