8 days after a deep plane face and neck lift with upper blepharoplasty, this patient is already back in the office for follow-up and doing very well.

He traveled from Chicago to address concerns with neck and jawline laxity, as well as upper eyelid hooding. His goal was clear: improved definition with a natural, unoperated appearance.

A result that once again reflects her natural structure! This patient came to us concerned about progressive heaviness along the lower face: jowling, marionette lines, and irregularity along the jawline that gradually blurred the transition between the face and neck.

Through a deep plane face and neck lift, combined with a brow lift and upper blepharoplasty, the deeper facial structures were repositioned to restore support where aging actually occurs. What you’re seeing here is not skin that has simply been tightened.

A comment we hear often from patients is:

The truth is, the neck is often the first area where we notice aging, but it doesn’t happen in isolation.

The SMAS and platysma muscles form a continuous layer that runs from the face into the neck. As we age, laxity in the lower face contributes to the appearance of neck sagging. That’s why addressing the neck alone may improve the contour temporarily, but for natural, harmonious results, the lower face often needs to be part of the plan.

Patients often ask this, and it’s an important one!

If you’ve had Sculptra, will it affect your results if you decide to have a facelift later? 💉

In most cases, the answer is no.

In my experience, common fillers like Sculptra or hyaluronic acid-based products are not typically an issue during surgery. They are often visible during the procedure and can be addressed as needed.

As for threads, they are usually removed during surgery. While they’re intended to provide lift, they do not create the same structural, lasting change that surgical techniques can achieve.

Every face is different, and treatment planning should always be thoughtful and individualized, especially when combining nonsurgical treatments with future surgical goals.

Preparing properly before surgery plays a major role in both your safety and your recovery.

Every patient at our practice meets with our team 3 to 4 weeks before surgery for a detailed pre-operative appointment (either in-person or virtually). During this visit we review everything, from medications to recovery expectations, so you feel fully prepared for the procedure and the healing process.

Here are a few of the most important guidelines we review with patients:

➡️ Two Weeks Before Surgery

• Stop taking NSAIDs such as Advil, Ibuprofen, Motrin, Aspirin, or Aleve unless cleared by Dr. Afrooz
• Discontinue ni****ne products including ci******es, va**ng, and ni****ne gum
• Avoid alcohol consumption
• Stop any unapproved supplements or vitamins
• Begin supplements such as Bromelain and Arnica, which may help minimize swelling and bruising

➡️ The Night Before Surgery

• Do not eat or drink anything after midnight
• Remove all permanent piercings and colored nail polish
• Remove false eyelashes and hair extensions
• Prepare soft foods for recovery such as soups, smoothies, or yogurt
• Focus on nutritious, high-protein foods to support healing

➡️ Morning of Surgery

• Brush teeth without swallowing water
• Do not wear makeup, lotion, perfume, contacts, or hair products
• Remove all jewelry and piercings
• Wear comfortable clothing such as a zip-up or button-down shirt
• Bring all medications we provided with you

These steps help minimize bleeding, reduce swelling, and create the safest possible surgical experience.

Botox can relax muscles.
But it cannot lift heavy tissue.

One of the most common questions patients ask is whether Botox can correct a low or drooping brow. The answer depends on why the brow is low in the first place.

In younger patients with naturally low brows, small adjustments with neuromodulators like Botox and Dysport can sometimes create a subtle lift by relaxing the muscles that pull the brow downward. But as we age, the situation changes.

Over time, the soft tissues of the forehead loosen and descend, which can create heaviness across the brow and upper eyelids. At that point, relaxing muscles with Botox may soften wrinkles, but it will not reposition the brow itself.

That’s where a brow lift becomes the more effective solution.

A brow lift restores the brow to a more natural, youthful position by addressing the underlying structure rather than temporarily weakening the muscles.

The goal isn’t a dramatic arch. It’s restoring balance between the forehead, brow, and upper eyelids.

Sometimes Botox is enough.

In the “before,” notice the degree of cervical skin redundancy and lower face laxity. Significant jowling, loss of jawline definition, and excess submental skin created a heavy neck contour.

Now look again. 👀

Her birthmark on the right cheek has shifted position, from the mid-cheek toward a more lateral location near the ear. We didn’t move the birthmark. We repositioned the underlying facial structures.

A deep plane lower face and neck lift releases and repositions the descended SMAS layer and soft tissue as a unit. By correcting tissue descent at its foundation and removing excess skin, we restore definition to the jawline and improve the cervicomental angle.

❌ This is not skin tightening.
✅ This is structural restoration.

The result: A cleaner neck contour, elimination of heavy jowls, and natural repositioning of facial landmarks.

Our patient is 49 years old and now 6 weeks out from her deep plane face and neck lift, healing beautifully.

One detail patients often ask about is incision placement. In her case, I chose a pretragal incision (placed just in front of the tragus) rather than hiding it behind the tragus. Why? Because she already had natural preauricular creases, which allow the incision to heal within those lines and blend seamlessly over time.

Incision design is never one-size-fits-all. It depends on skin quality, existing creases, degree of laxity, and ear anatomy.

At 6 weeks post-op, you can already see how well she’s healing. As swelling continues to resolve and scars mature, these incisions typically become very difficult to detect.
