Applications are now open!

The Older Adult Home Modification Program is targeted to help older adults (62+) age in place safely, by reducing fall risk, increasing accessibility, removing barriers to mobility, and increasing overall function within the home.

The program is open to homeowners and renters; landlords will have to sign an agreement for the modifications.

Each client will have an individualized assessment performed by an Occupational Therapist to determine which modifications will provide the best outcome. This program focuses on low-cost, high-impact modifications that will prevent falls, injuries, hospitalizations, or premature nursing home stays, to allow older adults to retain their independence while remaining in their homes longer.
