What is a Vascular Ultrasound and Why Do It?
Vascular ultrasound is a noninvasive and remarkably versatile procedure used to examine the circulation in the blood vessels of the body. It can also evaluate arteries or veins in nearly any part of the body.

When we refer to this exam as “noninvasive,” it means there is no need for needles or anesthesia and unlike other types of imaging, there is also no radiation or contrast dye.

How vascular ultrasound works
During a vascular ultrasound exam, sound waves pe*****te the organ or system of interest. The sound waves “bounce” off the blood cells as they move within the blood vessels. The sound waves then return to the ultrasound machine, where the technologist can view them on the screen and record them for the physician.

The key factor your doctor is examining is how fast the sound waves return to the ultrasound machine. This allows him or her to calculate the speed of blood flow in the vessel. If it is moving too fast, this indicates a narrowing of the vessel or a blockage.

Why perform vascular ultrasound
Vascular ultrasound helps visualize arteries or veins, including the blood vessels in the neck, abdomen, arms and legs. It aides in:

• Detecting clots in the vein (deep venous thrombosis, or DVT).
• Confirming chronic venous insufficiency, or leaky valves in the veins that may cause swelling, edema, and varicose veins.
• Evaluating blood flow in the arteries and veins of the body to detect the presence, severity and location of disease.
• Diagnosing the narrowing of arteries and pinpointing how severe the narrowing may be.
• Evaluating patients after a vascular procedure, such as a bypass or a stent.

𝗪𝗛𝗢 𝗜𝗦 𝗠𝗢𝗦𝗧 𝗔𝗧 𝗥𝗜𝗦𝗞?
People who smoke and/or have diabetes are at especially high risk. If you have risk factors for peripheral artery disease (PAD), get screened for PAD, even if you're not having symptoms.

Certain risk factors for PAD can't be controlled such as aging, personal or family history of PAD, cardiovascular disease or stroke. However, you can control the following risk factors:
 Cigarette smoking - Smoking is a major risk factor for PAD. Smokers may have four times the risk of PAD than nonsmokers.
 Diabetes mellitus - Having diabetes puts you at greater risk of developing PAD as well as other cardiovascular diseases.
 High blood pressure - It's sometimes called "the silent killer" because it has no symptoms. Work with your healthcare professionals to monitor and control your blood pressure.
 High blood cholesterol - High cholesterol contributes to the build-up of plaque in the arteries, which can significantly reduce the blood's flow.
 Obesity - People with a Body Mass Index (BMI) of 25 or higher are more likely to develop heart disease and stroke even if they have no other risk factors.
 Physical inactivity - Physical activity increases the distance that people with PAD can walk without pain and also helps decrease the risk of heart attack or stroke.

𝗪𝗛𝗔𝗧 𝗔𝗕𝗢𝗨𝗧 𝗧𝗛𝗘 𝗦𝗬𝗠𝗣𝗧𝗢𝗠𝗦?
While many people with peripheral artery disease have mild or no symptoms, some people have leg pain when walking (claudication). Claudication symptoms include muscle pain or cramping in your legs or arms that’s triggered by activity, such as walking, but disappears after a few minutes of rest. The location of the pain depends on the location of the clogged or narrowed artery. Calf pain is the most common location.

The severity of claudication varies widely, from mild discomfort to debilitating pain. Severe claudication can make it hard for you to walk or do other types of physical activity.

Peripheral artery disease signs and symptoms include:
 Painful cramping in one or both of your hips, thighs or calf muscles after certain activities, such as walking or climbing stairs (claudication)
 Leg numbness or weakness
 Coldness in your lower leg or foot, especially when compared with the other side
 Sores on your toes, feet or legs that won't heal
 A change in the color of your legs
 Hair loss or slower hair growth on your feet and legs
 Slower growth of your toenails
 Shiny skin on your legs
 No pulse or a weak pulse in your legs or feet

If peripheral artery disease progresses, pain may even occur when you're at rest or when you're lying down (ischemic rest pain). It may be intense enough to disrupt sleep. Hanging your legs over the edge of your bed or walking around your room may temporarily relieve the pain.

Between the constant activity, and summer vacations and the holidays, autumn is the perfect time to follow the post-treatment restriction on exercise and strenuous movement. Sending the kids to school each day can also allow you the opportunity to lie down and put your legs up following treatment.
2. 𝙄𝙣𝙨𝙪𝙧𝙖𝙣𝙘𝙚 𝙞𝙨 𝙚𝙖𝙨𝙞𝙚𝙧
By the time autumn comes around, many people have already met most or all of their insurance deductibles. However, October and November is not so late in the calendar year that you won’t have time to secure a vein treatment pre-authorization, if that’s what your insurance carrier requires. 𝗣𝗿𝗲𝗴𝗻𝗮𝗻𝗰𝘆 𝗮𝗻𝗱 𝗩𝗮𝗿𝗶𝗰𝗼𝘀𝗲 𝗩𝗲𝗶𝗻𝘀: 𝗪𝗵𝗮𝘁'𝘀 𝗮 𝗚𝗶𝗿𝗹 𝘁𝗼 𝗗𝗼?
Pregnancy does increase the chance that varicose veins will appear on your legs, but according to vascular specialists and obstetricians, there are things you can do to try to prevent them.

𝙒𝙝𝙖𝙩 𝘼𝙧𝙚 𝙑𝙖𝙧𝙞𝙘𝙤𝙨𝙚 𝙑𝙚𝙞𝙣𝙨?
Varicose veins are bulging, twisted veins that usually appear on your legs. They are generally blue or purple and you can feel them right under the surface of the skin. Sometimes they itch or even become painful.

Veins rely on one-way valves and leg muscles to help move blood back to the heart to get oxygen. When the valves stop working properly, blood can build up in the leg veins. This increased pressure forces the walls of the veins to bulge.
Pregnancy causes many changes in your body and some of these changes can cause varicose veins.
• 𝙋𝙧𝙤𝙜𝙚𝙨𝙩𝙚𝙧𝙤𝙣𝙚 – This hormone increases during pregnancy. It is essential for a healthy pregnancy, but it also causes the veins to relax meaning that blood does not move as efficiently through these relaxed veins. As the blood builds up, so does the pressure within the vein causing the vein to enlarge.
• 𝙄𝙣𝙘𝙧𝙚𝙖𝙨𝙚𝙙 𝙗𝙡𝙤𝙤𝙙 – The volume of blood in a pregnant woman’s body increases during pregnancy. More blood overall means there is more blood within the veins, and this increases the strain on the vein valves and walls.
• 𝙋𝙧𝙚𝙨𝙨𝙪𝙧𝙚 𝙛𝙧𝙤𝙢 𝙩𝙝𝙚 𝙪𝙩𝙚𝙧𝙪𝙨 – As your baby grows, the uterus pushes against the organs and blood vessels in the abdomen. Blood moves from the leg veins to veins in the pelvis, or lower abdomen, before moving back to the heart. The pressure placed on these pelvic veins by the uterus prevents blood from flowing out of the legs.
Now that you understand why you are more likely to get varicose veins when you are pregnant, let’s look at what you can do to help prevent them.

𝟳 𝗧𝗶𝗽𝘀 𝗼𝗻 𝗛𝗼𝘄 𝘁𝗼 𝗛𝗲𝗹𝗽 𝗣𝗿𝗲𝘃𝗲𝗻𝘁 𝗩𝗮𝗿𝗶𝗰𝗼𝘀𝗲 𝗩𝗲𝗶𝗻𝘀 𝗗𝘂𝗿𝗶𝗻𝗴 𝗣𝗿𝗲𝗴𝗻𝗮𝗻𝗰𝘆
• 𝙆𝙚𝙚𝙥 𝙈𝙤𝙫𝙞𝙣𝙜 – It can get harder to move around when you’re pregnant. You may already be exhausted. However, you need to move your leg muscles to help pump the blood out of your legs. If you have any concerns about walking, talk to your obstetrician to see if there’s a safe way you can keep moving while you are pregnant.
• 𝙒𝙖𝙩𝙘𝙝 𝙔𝙤𝙪𝙧 𝙒𝙚𝙞𝙜𝙝 – Increased weight puts more pressure on your veins. Of course, weight gain is expected, but you should try to avoid gaining it too quickly. Your obstetrician can help you determine how much weight you should be gaining in between doctor visits.
• 𝙏𝙖𝙠𝙚 𝘼 𝙇𝙤𝙖𝙙 𝙊𝙛𝙛 – Put your legs up, ideally higher than your heart, throughout the day. Use gravity to help the blood drain out of your leg veins. When you’re in bed, put a pillow under your feet to help the blood drain while you rest. If you stand a lot during the day, try to find time to take breaks so you can sit down and put your feet up.
• 𝙒𝙚𝙖𝙧 𝙈𝙖𝙩𝙚𝙧𝙣𝙞𝙩𝙮 𝙃𝙤𝙨𝙚 – Supportive pantyhose or thigh-high compression stockings work by applying pressure to the outside of your legs. This helps counteract the pressure within the veins and prevents swelling and bulging. You can put them on in the morning, but you should take them off when going to bed each night.

𝐕𝐚𝐫𝐢𝐜𝐨𝐬𝐞 𝐕𝐞𝐢𝐧𝐬 𝐃𝐨𝐧’𝐭 𝐀𝐬𝐤 𝐘𝐨𝐮𝐫 𝐀𝐠𝐞 𝐚𝐧𝐝 𝐃𝐨𝐧’𝐭 𝐂𝐚𝐫𝐞 𝐀𝐛𝐨𝐮𝐭 𝐘𝐨𝐮𝐫 𝐆𝐞𝐧𝐝𝐞𝐫

Nearly one quarter of Americans adults suffer from venous disease. If you include the number of people with other types of vein conditions, such as spider and reticular veins, that percentage goes as high as 85 percent. Varicose veins are not restricted to the older crowd, either. While it is more likely you will develop varicose veins in your golden years of life, this condition can strike at nearly any age. Even teenagers and young adults develop the bulging lines on occasion. That said, only about 10 percent seek treatment for it, and many of these individuals will not receive the correct diagnosis to their problem because of a lack of awareness about the disease according to the Society for Vascular Medicine.

Valves in a person’s leg veins keep blood moving against gravity and back toward the heart. Sometimes the vein walls weaken and valves become damaged. As a result, a person may develop venous disease, also known as chronic venous insufficiency. A person who has chronic venous insufficiency will experience a variety of symptoms created by the blood’s inability to travel out of the legs.

Symptoms of venous disease can vary a lot. It can be as simple as swelling, discomfort, and aching in the legs; or it can be bulging varicose veins that can be unsightly. In more extreme cases, there can be damage to the skin itself such as discoloration or sores that do not heal.

Many factors place a person at-risk for developing venous insufficiency. Females are more likely than males to develop the disease due to fluctuations in hormone levels. Individuals who spend a significant part of their day standing or sitting due to their occupation are also at high risk. Blood clots in the legs can also damage a person’s veins placing them at-risk for the disease. In addition, those who have a family history of venous insufficiency may develop it as well.

Many individuals can manage the disease initially by following conservative measures. The National Institutes of Health (NIH) suggests individuals take these steps:

• 𝘾𝙧𝙚𝙖𝙩𝙚 𝙘𝙤𝙢𝙥𝙧𝙚𝙨𝙨𝙞𝙤𝙣 – Wear compression stockings or socks to help aid in the upward circulation of the blood in the leg and to decrease potential swelling.

• 𝙆𝙚𝙚𝙥 𝙢𝙤𝙫𝙞𝙣𝙜 – Avoid sitting or standing for long periods. Even slight movement of your legs can help aid in circulation. Flexing (moving feet at the ankle joint up and down) the feet while sitting or shifting positions while standing can help alleviate pressure.

• 𝙄𝙣𝙘𝙧𝙚𝙖𝙨𝙚 𝙚𝙭𝙚𝙧𝙘𝙞𝙨𝙚 – Regular exercise can help strengthen leg muscles that aid in the constriction of the veins deep in the legs. It also can also help reduce the risk for being overweight, which can lead to the disease.

• 𝙎𝙚𝙚𝙠 𝙘𝙖𝙧𝙚 𝙛𝙤𝙧 𝙬𝙤𝙪𝙣𝙙𝙨 – Venous insufficiency can cause skin sores that are hard to heal. It is important to seek help from a physician to help care for any wounds that might come about due to the disease.

Venous disease can progress however, to a level where conservative measures are not enough and symptoms such as discoloration or thickening of the skin and skin ulcers form. In this case, medical intervention is available to provide relief. Many simple outpatient procedures are available that provide relief. About 95 percent of the time, a patient will see relief of their symptoms when we have accurately diagnosed their issue and responded with the appropriate therapy.

July is now upon us and you know what that means…summer heat is here too. Did you know that the aches and pains of summer could be signs of peripheral artery disease? While winter is often complained about, especially here in New York, summer brings its own difficulties. The increased amount of walking, hiking, sporting, and general physical activity leads to new pains. Some of these pains may simply be from lack of use caused by months of hibernating and staying out of the winter weather. However, some of these pains could be symptoms of a larger, more serious problem.

𝙎𝙞𝙜𝙣𝙨 𝙖𝙣𝙙 𝙨𝙮𝙢𝙥𝙩𝙤𝙢𝙨
If you are noticing pain in your legs and feet, the first thing you should do is take note of when the pain starts, when it stops, and how frequently it occurs. One surefire sign of peripheral artery disease is pain or fatigue that occurs when physically exerting yourself but dissipates quickly when you rest. Typically, exercise will cause pain near the end of – or after – exertion and may persist for a few days. If your pain does not fit this pattern, it may be time to speak with a professional.

Summer is finally here and that means excessive heat is just around the corner and so are reminders of staying hydrated and limiting your time in the sun. Those words of advice ring particularly true for anyone living with varicose veins.

Not only can heat increase the uncomfortable physical symptoms of varicose veins, but also varicose veins themselves can be embarrassing because of their thick and rope-like appearance.

Although the summer heat does not cause varicose veins, the increased temperatures can worsen symptoms. During summer and particularly hot spells, swelling or pain in the legs can be more noticeable. Repeated exposure to high temperatures – as we can see in the northeast in July and August – can cause blood vessels to expand and place more stress on veins’ valves. Heat dilates the veins, which causes the veins to expand. As the veins distend and fill, they become heavy and tired. Legs can feel tender, uncomfortable, painful, and crampy.

Additionally, as the sun’s rays weaken the skin’s upper layers, the skin loses moisture and elasticity. This makes both the skin and the veins just below its surface unable to flex and move properly. If the valves are weak or damaged, blood pools in the bottom of the vein, which causes the bluish rope-like bulgy appearance. This increased pressure in the veins can cause a sensation of heavy aching, pain, throbbing, or swelling in the legs.

𝗪𝗵𝗮𝘁 𝗰𝗮𝗻 𝘆𝗼𝘂 𝗱𝗼?

𝙎𝙩𝙖𝙮 𝙘𝙤𝙤𝙡
Swimming, especially in cool water, can help blood circulation. Just be sure to choose hours for swimming before or after the heat of day (11 a.m. to 3 p.m.), or you can swim indoors. Sit in the shade. Hang out inside where it is air conditioned. No AC? This is a good time to visit the mall, your library or a favorite coffee shop.

𝘾𝙝𝙤𝙤𝙨𝙚 𝙛𝙡𝙖𝙩 𝙨𝙖𝙣𝙙𝙖𝙡𝙨 𝙤𝙫𝙚𝙧 𝙝𝙚𝙚𝙡𝙨
When you wear flat shoes your calves have to work harder, which in turn promotes muscle activity unlike heels. The bottom line is that working the calf muscles keeps your blood flowing in your legs.

𝙀𝙭𝙚𝙧𝙘𝙞𝙨𝙚
Walk, bike, swim and surf your way to leg health. A walk along the beautiful lake and river beaches in New York is a great place to work your leg muscles. Cooling exercises like swimming, surfing and brisk evening walks are best, because the coolness helps keep the blood vessels from dilating, and exercise keeps the blood circulating.

𝙐𝙨𝙚 𝙨𝙪𝙣𝙨𝙘𝙧𝙚𝙚𝙣
Varicose veins are one more reason to apply sunscreen. Thinner skin is more likely to develop spider veins. Always apply sunscreen before heading out into the summer sun and cover up, too.

𝙃𝙮𝙙𝙧𝙖𝙩𝙚
Drink plenty of water. There are many reasons to keep sipping water all day. Staying hydrated has many benefits such as keeping veins plump and blood circulating properly. Dehydration can cause the tissues in your legs to swell and your muscles to cramp and ache. Limiting or avoiding alcoholic beverages might help lessen the chance of dehydration and increased varicose vein symptoms too.
