Attention all CWH patients:
As we transition to our new system, we do request your patience as we are getting fully integrated. Updating your patient profile located in the "My Health" portion of the portal will help to decrease the intake time for your appointments; and signing up for the Klara messaging app will allow you to be able to contact us without having to log fully into the Modmed portal. Please be mindful that during this time as things are switching over we are experiencing unavoidable delays. It is also helpful should you have an upcoming appointment that you contact us to ensure you are in the new system where you were initially scheduled, as some visits are not crossing over as expected. Again, we appreciate your continued patience in regards to this matter.

Attention:

Exciting News! CWH will be switching to a new EMR system, ModMed. During this time as we are learning, we will be operating on a shortened clinic day 9/23/25 - 9/25/25 and there will be no afternoon clinic those days from 12-4pm. This also means we will not be available by phone during that time, however you may always still reach out if needed through your patient portal as usual, but if you have urgent or emergency needs we would advise you to go to your nearest emergency department. We thank you for your patience as we make this transition and look forward to offering our patients more services through this system. As always, thank you for being a part of the CWH family!
