Good morning,
I would like to provide an update regarding Care Coordination services.
Clients are still required to submit appointment slips at least 24 hours in advance and to communicate directly with their Care Coordinator. For your convenience, a list of THIP staff contact numbers is included below.
At this time, we ask that you call and confirm all upcoming appointments with your Care Coordinator to avoid any lapses or miscommunication.
Due to unforeseen circumstances we must prioritize clients with higher equity needs. Priority will be given first to clients with the greatest needs, particularly those requiring life-sustaining treatments. These may include but are not limited to pediatrics, dialysis, and chemotherapy treatment.
