Remember. If you need medical records refer to the letter that was sent out. Call Carr Family Chiropractic at 337-993-2277. They are doing myself and my patients a favor by being the medical records custodian. Please be kind and let them know ahead of time the information you need. I have your clinical notes with me and have to send it to them. They will charge for copies and their time. If you need a copy of an MRI or x-ray report please contact the facility where you had the study completed, as this is quicker and does not cost you.
