Great example of why looking at a blood smear is so important!
🔬Things that would've been missed if a blood smear wasn't evaluated in this case:
🩸Abundant spherocytes and ghost red blood cells
🩸20% nucleated red blood cells, misclassified by the instrument as lymphocytes --- which caused a false lymphocytosis on the automated CBC. Lymphocytes are actually decreased in this patient
🩸Left shifting and toxicity within the neutrophil series - signifying an inflammatory leukogram

Pretty images from a suspected keloidal fibrosarcoma. Eosinophilic bundles of hyalinized collagen were scattered throughout (classic for keloidal lesions) with many slender spindle cells that exhibited mild to occasionally moderate pleomorphism and rare binucleation. Histopathologic evaluation was recommended for further assessment and differentiation between a keloidal fibroma and keloidal fibrosarcoma.
