📢 PSA: Understanding Advance Directives in Texas

As we get older, making decisions about how to communicate our healthcare wishes to loved ones and medical providers can feel overwhelming. There are several different types of Advance Directives in Texas, and each serves a unique purpose.

The Texas Health and Human Services website provides all official forms at no cost:

The available forms include:

• Declaration for Mental Health Treatment (DMHT)
• Directive to Physicians and Family or Surrogates (Living Will)
• Medical Power of Attorney (MPOA) – Designation of Health Care Agent
• Statutory Durable Power of Attorney (SDPOA)
• Out-of-Hospital Do-Not-Resuscitate (OOH-DNR) Order

⚠️ Important Reminders:

✔️ A Medical Power of Attorney (MPOA) only becomes effective when you are no longer capable of making your own medical decisions. It does not allow someone to immediately make healthcare decisions for you simply because the form is signed.

✔️ A DNR signed in a hospital does NOT automatically apply in nursing homes or other community settings. For those settings, an Out-of-Hospital Do-Not-Resuscitate (OOH-DNR) Order must be signed to ensure your wishes are honored.

Advance care planning is one of the most important conversations you can have with your family. Taking time now to complete the appropriate forms can provide peace of mind for everyone involved.

We are incredibly thankful to Alfonso Venegas for his generous $30,000 gift to the Dallam Hartley Counties Healthcare Foundation in support of our More Than a Home capital campaign to build a new long-term care facility for Dalhart.

Alfonso is a dedicated, involved, and passionate member of our community, and this gift reflects the heart of Dalhart—neighbors investing in neighbors and believing in the future of where we live. Support like this makes a meaningful difference and helps move this much-needed project forward.

As Alfonso shared:

💛 Celebrating 20 Years of Legacy Assisted Living 💛

Yesterday was a truly special afternoon as we gathered with residents, families, staff, and community members to celebrate 20 years of care, memories, and meaningful connections at Legacy Assisted Living.

Guests enjoyed an afternoon filled with shared stories, laughter, and reflection—exploring scrapbooks, original renderings, newspaper clippings, and the rich history of Legacy. One of the most meaningful moments was opening a 10-year time capsule, revealing memories, photos, and documents from the past that reminded us just how far we’ve come.

We are incredibly honored to have served our residents and community for the past 20 years, and we are excited to look ahead to the next 20. Thank you to everyone who joined us to celebrate and to all who have supported Legacy along the way. Your love and support mean more than words can say. 💙

Join us for a Town Hall to learn more about More Than a Home, our capital campaign to build a new Nursing Home for Dalhart—made possible by the generous $10 million pledge from Louise and Gene Rahll.

This Town Hall will share why this new facility is so greatly needed, how it supports the long-term sustainability of Dallam Hartley Counties Hospital District, and how you can get involved through support, giving, and spreading the word. It’s now up to us as a community to raise the remaining funds needed to make this project a reality.
