Medical abuse in healthcare, also known as health care fraud and abuse, is the intentional misuse of resources, funds, or services in a healthcare setting. Examples of medical abuse include:
1) Billing for services not provided: Submitting claims for services that were not performed or used
2) Falsifying claims: Falsifying medical records or claims
3) Misrepresenting services: Misrepresenting the dates, frequency, duration, or description of services provided
4) Billing for unnecessary services: Billing for services or items that are not medically necessary
5) Unbundling: Separating services that should be billed together into multiple codes to increase reimbursement
6) Exploiting patients: Entering false diagnoses into a patient's medical records to submit bogus insurance claims
7) Overcharging: Charging more than the appropriate amount for services or supplies
8) Misusing billing codes: Using billing codes in an improper way to increase reimbursement
9) Falsifying eligibility: Falsifying a patient's eligibility
