People with unilateral cleft lip have an asymmetrical and crooked nose. An uneven appearance of the nostrils and nasal tip are stigmatizing and draw unwanted attention that can impact self esteem. Twenty years of research, innovation, and clinical experience continue to refine my approach to cleft care.

This publication of mine from 2024 in The Cleft Palate–Craniofacial Journal explores alar asymmetry in unilateral cleft lip nasal deformity, analyzing shape, volume, and axis rotation in skeletally mature patients.

The ala is the fleshy part of the nostril. In many cases the ala is significantly smaller on the side of the cleft lip and that contributes significantly to an uneven appearance of the nostrils. This asymmetry is often overlooked by surgeons and left untreated. I hope this paper will help improve outcomes for all patients undergoing cleft rhinoplasty. 🌟

I frequently see people who have had an otoplasty but remain unhappy with the appearance of their ears. In many cases these individuals have a focal excess of cartilage in one portion of the ear called the scapha. This excess scapha can cause residual prominence of the ear and/or the ears can look disproportionately large. This patient was unhappy with the large size and pointed appearance of his ears after previous otoplasty. He is shown before and just 6 weeks after ear reduction surgery (scapha reduction). His ears are now more proportionate and have natural looking contours.

Ear reduction is a versatile procedure that can improve the appearance of the ears caused by focal excess of cartilage in the ear and it can also be used to reduce the size of excessively large ears (macrotia). This approach to refining the appearance and proportions of the ears focuses on targeted removal of excess cartilage that causes the ears to be disproportionately large or stick out farther than normal. Ear reduction can be combined with otoplasty techniques to improve the contours of the ears when indicated by the patient’s anatomy.

In experienced hands, there is minimal visible scarring, and the results speak for themselves. This lovely patient felt that her ears were disproportionately large. She did not have any anatomic features that made her a good candidate for a traditional otoplasty. She is shown before and just 6 weeks after ear reduction surgery. She now has a natural and proportionate appearance of her ears, and she is beyond happy with her results.

Swelling after rhinoplasty affects both the appearance of the nose and the ability to breathe. The resolution of swelling can vary from person to person, but there is a fairly predictable pattern observed. 1-2 weeks after surgery swelling blunts the fine details of the bridge and nasal tip. Significant nasal congestion makes it hard to breathe through your nose similar to a head cold. 2-4 weeks after surgery the swelling is significantly reducing and nasal congestion is significantly improved. By 6 weeks after rhinoplasty patients are breathing comfortably and feeling back to normal. There is trace swelling left in the nasal tip, but the appearance of the nose is very close to the final result. 1 year after the rhinoplasty the trace swelling in the nasal tip resolves, revealing the delicate features of the nasal tip.

For many children and adults, otoplasty is a life-changing procedure that helps restore confidence and comfort.

Our youngest patients are treated at Park Cities Surgery Center in Snider Plaza, with anesthesia provided by, , a board-certified pediatric anesthesiologist.

Adult patients can choose the setting that fits them best: general anesthesia at the surgery center or local anesthesia in our office procedure room.

For people with a unilateral cleft lip the nose is asymmetrical. The most common concern these patients express is an uneven appearance of the nostrils and nasal tip. This patient’s primary concern was the uneven appearance of his nostrils. He also wanted to improve his profile. When planning for and performing cleft rhinoplasty I am not only trying to address the stigmatizing features of the nose that get unwanted attention. I am also trying to make the best version of the patient’s nose that will fit their face and look natural, just as I do for my cosmetic rhinoplasties. In this case, the nostril symmetry was improved and the balance between the bridge of the nose and the nasal tip was refined. The photos are before and 6 months after cleft rhinoplasty. strong

This lovely patient came to me with a history of cleft lip and a previous rhinoplasty. She was unhappy with the lack of definition and disproportionately large size of her nose. She felt that her nose just didn’t fit her face. She is shown before and 6 months after her cleft rhinoplasty. Her nose is more symmetrical and defined and her profile looks amazing.
