Our mission is to provide exceptional and loving care while improving the quality of life for persons and their families faced with a life-limiting illness. We understand this is a difficult journey for your loved ones and families. That is way we take this responsibility very seriously and make every effort to care for your loved ones as if they were our own family. There is not enough education out there about hospice so when given the chance, we can do educations in the patents home, at Nursing Facilities, Assisted Living Facilities or in the community. We can meet with patients and/or families at their convenience and we work around their schedule so we can provide education to them on what all is involved when they are on hospice and what United Plus Hospice can do to help in their time of need. We mold our care to what each patients needs are. We have RN's that provide excellent patient care, as well as CNA's who can assist patients with various personal care needs, Social Workers who can help with resources and who are also here for emotional support for the patient and their families, as well as a Chaplain who is here for additional support to our patients and families. We also have a Bereavement Coordinator here to help families and friends through the bereavement process.
