ISMP reports a tragic incident where a patient received the wrong medication, leading to their untimely death. The patient required GoLYTELY but instead received NaturalLyte, which sounds and appears similar. Unfortunately, barcode systems in medication administration are not always foolproof, and it seems that a staffing shortage in the ICU on that day may have contributed to the error. The ISMP recommends escalation processes if a barcode on a medication does not scan successfully. This serves as a heartbreaking reminder of the importance of being vigilant in medication administration and the need for reliable systems to prevent errors.
