Be diligent in your choice when selecting your vendors.

APCM can be partnered in pharmacy, but only with a collaborative agreement similar to CCM with partnering clinics that manage the patient's whole life.

CMS States the following:

Starting January 1, 2025, you can bill for APCM services if:

✅ You’re a physician or non-physician practitioner (NPP), including a nurse practitioner (NP), physician assistant (PA), or clinical nurse specialist (CNS)

✅ You’re responsible for ALL of your patient’s primary care services

✅ You’re the focal point for ALL of your patient’s needed health care services

✅ You’ve gotten either written or verbal consent from your patient

APCM service codes are primarily for primary care specialties, like general
internal medicine, family medicine, geriatric medicine, or pediatrics.

NOT TO MENTION YOU HAVE TO MEET 13 DIFFERENT METRICS TO BILL!

This said --> We are working towards Pharmacies taking a bigger role clinically with patient care and be able to bill for services like these on their own. Until then..... protect yourself from fraud, waste, and abuse. ✌
