Wednesday Evening Sports Injury Clinic – Sideline Orthopedics & Sports
Does your athlete have a NEW sports-related injury? We’re here to help!
📅 WHEN:
Every Wednesday evening, 5:00 – 6:00 pm
Students can arrive at 4:30, first come, first serve.
(Check-in by 6:00 pm Doors are locked after 6)

WHO CAN ATTEND?
• School-aged athletes with a NEW sports-related injury (excluding head injuries).
• Note: Concussions cannot be evaluated.
• We can only treat one injury per visit.

WHAT TO EXPECT?
• X-rays and initial exam by our provider are included.
• Bring insurance cards and/or school claim forms for any additional studies (e.g., MRI).
• If follow-up care is needed, an appointment will be scheduled during regular clinic hours (billed accordingly).

PARENT/GUARDIAN CONSENT:
• A parent or guardian must attend if the athlete is a minor.
• Coaches or Athletic Trainers may bring a signed authorization to treat a minor form.

REHAB & REFERRALS:
• Rehab may be provided by the school Athletic Trainer when applicable.
• Local physical therapy (PT) may be prescribed if needed (billed accordingly).
• Some athletes may require a primary care referral for further treatment.
