Home Health RN Case Manager immediate opening.
Position Summary:
The Home Health Nurse provides skilled nursing care to patients in their homes under the direction of a physician. The nurse is responsible for assessing patient needs, developing and implementing care plans, providing education to patients and families, and coordinating services with the interdisciplinary healthcare team to promote optimal health outcomes.
Essential Duties and Responsibilities
• Perform comprehensive patient assessments and document findings accurately.
• Develop, implement, and update individualized patient care plans.
• Administer medications and treatments as prescribed by the physician.
• Monitor patient progress and report changes in condition to the physician and care team.
• Provide wound care, IV therapy, catheter care, and other skilled nursing services as ordered.
• Educate patients and caregivers regarding disease management, medications, treatments, and home safety.
• Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
• Maintain accurate and timely clinical documentation in accordance with agency policies and regulatory requirements.
• Participate in case conferences and interdisciplinary care planning.
• Ensure compliance with all federal, state, and agency regulations related to home health services.
• Promote patient independence and quality of life through compassionate, patient-centered care.
Qualifications
• Current and active Registered Nurse (RN) license in the state of employment.
• clinical nursing experience and/or home health experience preferred).
• Strong clinical assessment and critical thinking skills.
• Ability to work independently and manage time effectively.
• Excellent communication and patient education skills.
• Valid driver's license and reliable transportation.
Physical Requirements
• Ability to travel to patient homes.
• Ability to lift up to 25–50 pounds as needed.
• Ability to stand, walk, bend, and perform nursing procedures.
Work Environment
Patient homes in a variety of community settings. Exposure to typical home health environmental conditions.

Executive Assistant to CEO/HR Manager immediate opening.

The purpose and goal of the Executive Assistant I HR manager is to enhance the effectiveness of the CEO and manage various HR responsibilities. The primary responsibilities will include managing calendar, handling correspondence and maintaining smooth communication between the CEO and various departments and external stakeholders. HR responsibilities would include employee orientation, employee health insurance enrollment and other various responsibilities.

This role will require exceptional organizational skills, attention to detail, discretion and ability to multitask effectively.

Duties and Responsibilities:

Providing administrative assistance, such as writing and editing e-mails, drafting memos, and preparing communications on the executive’s behalf
Maintaining comprehensive and accurate records
Performing HR related responsibilities
Organizing meetings, including scheduling and sending reminders.
Answering phone calls in a polite and professional manner
Welcoming visitors and identifying the purpose of their visit before directing them to the appropriate department
Managing the executive’s calendar, including making appointments and prioritizing the most sensitive matters
Requirements and Qualifications:

Minimum of associate’s degree, bachelor’s degree preferred
Proven 3 to 5 experience as an executive assistant or similar role, preferably supporting senior executives or C-suite professionals
Minimum of 1 to 2 years in HR related responsibilities
Possess quality computer skills and good proficiency in Microsoft Office Suite and other relevant software applications
Discretion and confidentiality in handling sensitive information and dealing with confidential matters
Self-starter with ability to work under minimal guidance and direction
Strong problem-solving skills, resourcefulness, creativity and can-do attitude
A willingness to work outside regular business hours as needed
Time management and ability to meet deadline
