Prehospital blood is having its moment. And the momentum is real.

New study in Prehospital Emergency Care just mapped the national landscape, and there's a lot to be excited about.

13 states plus DC now allow field-initiated blood transfusion in their statewide EMS protocols. That number was near zero just a few years ago.

The wins buried in the data:
✅ 69% of those states allow transfusion for medical emergencies, not just trauma.
▶️ Obstetric hemorrhage, GI bleeds, ruptured AAAs.
✅ 69% allow pediatric transfusion. Our smallest patients no longer have to wait for the ED doors.

Here's the real opportunity this paper surfaces. We're at the inflection point. The NAEMSP position statement dropped in April 2025. The Dorsett data. The Braverman data. The Pusateri data. The playbook is written.

For the 37 states still building their protocols, you're not behind. You're early. You get to learn from the states that went first. You get to launch with low titer group O whole blood or PRBCs + plasma, composite physiologic indications, and pediatric inclusion from day one.

Moving a patient on ECMO sounds like the riskiest transport you'll ever do.

But this study of 151 ECMO transports by specialized critical care teams flips that assumption.
