Congenitally Missing Lateral Incisors 🦷

The lateral incisor region can be one of the most challenging areas for implant placement. Limited bone development, a narrow space, and the high esthetic demands of the smile zone mean every millimeter counts. Achieving the ideal emergence profile, gingival symmetry, and papilla support often requires careful planning and a staged approach with orthodontics, bone grafting, and soft-tissue enhancement.

Here we’re treating a young female in her 20s with a congenitally missing #10. The space was narrow and the facial bone width was insufficient for implant placement. We performed guided bone regeneration using a combination of Geistlich Bio-Oss, Creos Allograft, and platelet-rich fibrin. Graft immobilization with a collagen membrane and tacs couple with tension free closure. She’s now 5 months post-op and nearly ready for implant placement. 👏

How important is radiographic surveillance? 🦷📸

If a patient elects not to remove fully impacted third molars, periodic radiographic surveillance every 2–3 years is recommended—or sooner if symptoms or pathology involving adjacent teeth develop.

The radiographs shown are from a 37-year-old patient who initially deferred third molar extraction due to proximity to the inferior alveolar nerve. Over time, progressive radiographic changes were observed around tooth #17, with early signs possibly evident in 2022 and a definitive coronal cystic lesion identified in 2025. The patient subsequently underwent surgical extraction and enucleation of the cystic lesion. Tissue diagnosis: dentigerous cyst.

This case is a reminder that wisdom teeth can cause problems silently over time. Early evaluation and proactive care can make a big difference. 💪
