Our first of several dates for sports physicals kicks of next Tuesday, May 21 at Sangamon Valley High School from 3-5pm.

We have also added one more date. August 2 will be our final date for sports physicals and will be held at the SIU Family Medicine facility at 102 W. Kenwood in Decatur.

We will have the IHSA sports physical forms onsite and you are welcome to show up to any of the dates even if it’s not the school your student attends. We will make sure the physical form gets to your school.

Over the past couple weeks we have seen some very serious concussion cases at both local and national levels. Proper recognition and management of concussions is vital to prohibit further injury with potential catastrophic consequences.

Dr. Susan Wu is Decatur's only Credentialed ImPACT Consultant and utilizes the ImPACT program as a valuable tool in assessing recovery from concussions and returning patients back to school, work, and sport.
