Join our team at Adams Woodcrest, Adams Heritage, and Adams Memorial Hospital! As a Care Navigator, you’ll coordinate admissions and transfers, ensure resident safety and comfort, and work closely with families, physicians, and insurance providers.

✅ Medical coverage starting Day 1

🕗 40 hrs/week – First Shift (8:00 AM – 4:30 PM)

Join our team and make a real impact by providing person-centered care to our residents. As an RN or LPN, you’ll:
✅ Plan, implement, and document nursing care
✅ Assist physicians and conduct patient tests
✅ Administer medications as permitted
✅ Educate residents and families while promoting independence
✅ Foster a safe and comfortable environment for all
