24-hour hospice care that addresses the needs of both patient and family There are times when a terminal illness cannot be adequately managed at home.
CHP offers inpatient hospice centers in Defiance and Van Wert that specialize in pain and symptom management so that patients can return to wherever they call home. What makes CHP Inpatient Hospice Centers different is that all hands-on care is provided by specially trained hospice nurses. We give 24-hour care that ministers to physical, emotional and spiritual concerns. The unique design of our 12-bed centers addresses the needs of the whole family. All rooms are private suites with immediate access to skilled nursing care and an on-site social worker. Families have access to a chapel, kitchen, library, plus showers and overnight accommodations. There is space for family to gather and quiet places to be reflective. Care is available on a short-term or long-term basis.

đ¶Yesterday, CHP Hospice physicians - Dr. Kevin Park and Dr. Beth Besaw - recorded our first podcast with Wendy Gericke of CHP as moderator. Their discussion covered topics like: When should someone start thinking about hospice care? What are the signs a family should look for? Do most people wait too long? And, what happens if you start hospice earlier vs. later? We will release the podcast soon...stay tuned. đ»

March is National Social Work Month, and today we want to take a moment to recognize our own Inpatient Hospice Social worker, Laurie Hockenberry.
Whether itâs offering a listening ear, helping navigate complex decisions, or simply being present, her impact reaches far beyond what can be measured.
In some of lifeâs most difficult moments, social workers show up with compassion, strength, and unwavering support. They advocate for our patients, guide families through unimaginable transitions, and provide comfort when words are hard to find.
Hospice isnât just a job, itâs a calling. We are often the last to witness a final breath, listening for one more heartbeat even when there is none. When people hear, "hospice," their smile sometimes fades into sadness. But hospice can really be a gift to a patient or a family.
Because hospice isnât about giving up, itâs about showing up.
While hospice is end-of-life care, it is so much more than that. Itâs caring for patients and their families spiritually, emotionally, and physically. Itâs bringing comfort in moments that feel overwhelming. Itâs creating peace when life feels uncertain.
Being present, holding a hand, brushing someoneâs hair, whispering words of reassurance, reminding them they are not alone; it matters more than words can express. Sometimes the hospice team becomes the only family a person has in those final moments.
