Sometimes the first step to a refreshed look is dissolving what’s not working. 💉 This 50-year-old patient had Hylenex to dissolve old filler that was weighing down her under-eye area—you can even see it here in her photo. Once that was cleared, we were able to achieve a brighter, natural result with upper + lower blepharoplasty, fractional laser for the lower lids, midface lipografting, and a TCA peel. 👀

Under-eye hollows can age the face more than wrinkles.🤔
They create shadows, flatten the midface, and make
you look tired even when you're not.
Lipografting is one of my favorite ways to restore this area.
Unlike filler, it uses your own tissue to naturally replace lost volume and blend the under-eye into the cheek and temple for a softer, younger contour.

After years of under-eye filler, Iliana was ready for something long-term. 🙂‍↕️👏
Instead of topping off again, we went for the ultimate refresh: dissolving her old filler, removing bulging fat pads, and restoring volume with her own purified fat.

Why the switch? 🧐
Filler under the eyes can remain for many years (her treatment was about 6–7 years ago) and very often creates a visible swelling under the skin because it attracts water.
Fat grafting will last for many years, is natural looking and creates a smooth lid–cheek junction without the puffiness.

Do you have to remove fat for an upper blepharoplasty?
Not always! The goal is to refresh your eyes, not hollow them out. If you don't have significant fat bulging in the inner corner, we leave it alone to maintain a natural, youthful curve. For most of my patients, I remove only excess skin (and a touch of fat if needed) to create a smooth, lifted contour-never a skeletonized look. What's the right age for a facelift - 40? 70?
Here's the truth: The deep plane facelift technique I use is similar for patients in both age groups, but not exactly the same.
What really changes is the amount of skin removed - not the foundational approach.
Younger patients may see subtle, refreshed results. Older patients often have more dramatic transformations.
Age alone doesn't determine candidacy — it's about facial laxity, anatomy, and your personal goals.
Recovery? Roughly the same.
Longevity? Excellent in both.

We're dissolving old filler before Iliana's lower bleph + lipografting in 2 weeks.
🤔Why? Because fat is more permanent, more natural-looking, and doesn't leave that swollen, puffy effect over time.

Lip rejuvenation is often overlooked when talking about surgical procedures, however it can make the biggest impact on the face.
As we age, the length between the base of the nose and upper lip elongates, often resulting in a loss of upper tooth show.
To help address this area, a lip lift and corner lip lift, as seen in this before and after, can help create a more youthful appearance to the lower face. The lip lift is done by creating an incision below the base of the nose and removing excess tissue, which helps elevate and shorten the distance. The corner lip lift, done at the corners of the upper lip, help roll the outer portions of the red lip out, creating an even more youthful and fuller appearance to the upper lip.
This is one of my favorite "cherry on top" procedures to perform. e
