As we prepare for Christmas, we got an assessment to code from the North Pole. It seems that, in November, during a pre‑flight prep, Santa did a post‑cookie strut on an icy roof and sprained that right ankle. Because of all the cookies he is a Type 2 diabetic on insulin with mild neuropathy to both extremities. Homebound status is documented (requires use of crutches and has limited ambulation ability due to recent injury/safety risk to leave home). He’s healing well and will be discharged from home health, no longer homebound, by Christmas.

Codes that apply (short & merry):

S93.401D - Sprain, unspecified ligament, right ankle, subsequent encounter
W00.0XXD - Fall on same level due to ice and snow, subsequent encounter (external cause)
E11.42 -Type 2 DM with diabetic polyneuropathy
Z79.4 - Long‑term insulin use
Z74.09 - Other Reduced mobility (if documented)

Tip: Injury first, external cause next; keep the 7th character “D” for subsequent encounter and verify against current ICD‑10‑CM & payer guidance.

MAC Legacy coding experts since 2004 and a trusted industry resource (we were providers first), wishing you tiny denials and mighty comorbidity adjustments.

Planning Your Q1 Mock Survey Starts Now

The best way to prepare for survey is to plan ahead and a Q1 Mock Survey with the MAC Legacy team is one of the most effective ways to do just that. Our mock surveys are designed to mirror real survey conditions, helping your team identify strengths, uncover gaps, and build confidence before surveyors arrive. By planning early in Q1, you give your staff time to address findings, reinforce education, and walk into survey season feeling prepared, not pressured.

Leadership Lesson with Marcylle Combs - Looking Ahead to 2026
As we look toward 2026, I’m reminded that leadership isn’t about having all the answers it’s about being willing to learn, adjust, and keep showing up. The past year reinforced how much this work matters and how much we rely on one another in home health and hospice. None of us do this alone, and the best leaders I know are the ones who listen well, stay grounded, and lead with both confidence and compassion.

My hope for 2026 is simple: that we continue to lead thoughtfully, support our teams well, and never lose sight of the patients and families at the center of what we do. Progress doesn’t always come from big moves, sometimes it comes from steady leadership, strong relationships, and a commitment to doing the right thing even when it’s hard. I’m grateful for the opportunity to learn alongside so many dedicated leaders, and I’m looking forward to what the year ahead holds.

Spreading Cheer & Team Spirit This December!
"The best way to spread Christmas cheer is singing loud for all to hear." – Buddy the Elf
This month, our team came together for time that was full of giving and fun:

Leadership Lesson with Marcylle Combs - Buddy the Elf and Leadership
At our recent quarterly team meeting, we decided to have a little fun with the holidays and had a full-on Buddy the Elf-themed Christmas party! There were snowflakes, candy canes, and plenty of laughter. But in the middle of all the festive chaos, I shared a few leadership lessons inspired by Buddy himself.

Now, Buddy may not be your typical leadership expert, but his story is full of wisdom that speaks to us in home health and hospice.

Here are three lessons worth remembering:
• Celebrate Authenticity
Buddy never tried to be anyone but himself, even when he didn’t quite fit in. As leaders, authenticity matters. Your team and your patients need to see the real you. When we lead with honesty and transparency, we build trust, and trust is the foundation of great care.
• Find Joy in Bringing Joy
Buddy’s mission was simple: spread Christmas cheer. In our world, joy looks different, it’s a comforting word to a family, a smile during a tough day, or going the extra mile for a patient. Leaders set the tone. When we model joy, even in challenging times, it ripples through our teams and into the homes we serve.
• Stay Hopeful for Belief
Buddy never stopped believing, in Santa, in family, in the goodness of people. In home health and hospice, hope is powerful. We lead teams who face difficult realities every day. When we stay hopeful believing in our mission, our people, and the difference we make, we inspire others to keep going.

As we wrap up this year, let’s take a page from Buddy’s book: lead with authenticity, bring joy wherever you can, and never lose hope. After all, the best way to spread cheer, whether at Christmas or any time, is to live it out loud.

Since 2004, MAC Legacy has been a trusted partner for home health and hospice agencies across the country. We didn’t enter the coding space, we helped define it.

Our certified, tenured coders work under clinical oversight, ensuring every chart is accurate, compliant, and ready to support clean claims. We offer 24-hour turnaround, and agencies can add OASIS review and Plan of Care review for a complete documentation strategy.

Why Agencies Choose MAC Legacy:
• Certified coders with deep experience
• Clinical oversight for added accuracy
• Optional OASIS and Plan of Care review
• 24-hour turnaround
• Scalable support for agencies of all sizes
• Responsive, real customer service, we listen, we follow through, and we stand behind our work

We come from the field, our roots are in care delivery, not just consulting. That’s why agencies trust us to be more than a vendor. We’re a partner who understands what’s at stake.
