RMHS is a nonprofit organization that offers person-centered case management and direct services to improve the health, self-sufficiency, and overall quality of life for children, adults, and veterans in Colorado. Currently, we have the privilege of serving:

Who: Denver families with children ages 5 and under

As a Youth Team Clinical Care Manager and Mentor in the Community Transitions Program, Noah plays a critical role in supporting youth and families, helping them navigate challenges and access the care they need. When asked what people should know about working at RMHS, Noah shared that every day brings real opportunities to make a difference. If you support adolescents or young adults with I/DD, there is still time to register for the Denver START team's upcoming Clinical Education Training on January 28.

This session will focus on the critical transition from pediatric to adult systems for individuals with I/DD.

Many crises during this transition are preventable with early planning. A common myth is that turning 18 automatically means “independence.” In reality, support needs often increase during this period.

Participants will learn how to identify risks, build readiness, and support continuity of care using a START-informed, person-centered approach.

This training is open to care coordinators, clinicians, educators, case managers, transition specialists, and other professionals who support adolescents and young adults with I/DD.
