System Navigators connect Iowans with services and advocate on their behalf. The team acts as a bridge to support when it’s needed most.

A parent, child, and elderly mother were seeking shelter and connected with our System Navigation team. The family was initially turned away from their local shelter due to not meeting residency requirements. A System Navigator stepped in and clarified that the family had already lived in the area for several months—a vital step that helped secure their spot at the shelter. Once the family’s shelter was confirmed, the System Navigator arranged transportation for them the following day.

By taking the time to understand and explain the situation, the System Navigator ensured that the family stayed together. If you or a loved one find yourself in a similar situation, please call Your Life Iowa at (515) 581-8111 and ask for a System Navigator.

Kyle Briese, Chief Financial & Analytics Officer at the Iowa PCA, and Tom Bowman, CEO of Community Health Care, Inc., were recently interviewed by WQAD about the Iowa PCA's recent $523,750 AIM-AHEAD grant award from the National Institutes of Health.

The project will support development of a “fairness-aware” artificial intelligence and machine learning (AI/ML) model designed for Iowa’s community health centers—helping care teams more quickly identify and support patients with medically complex, high-risk conditions and ultimately improve health outcomes.

Since we launched statewide System Navigation services, we’ve seen countless examples of Iowans being connected to care.

One recent example: an Iowan being discharged from out of state and needed immediate support. While one system navigator coordinated with the individual’s social worker, others across the district called shelters until a placement was found.

This teamwork is possible thanks to our strong network of system navigators and their commitment to helping Iowans access care—no matter where they live or their insurance status.
Support is just one call away. Call Your Life Iowa at (515) 581-8111 and ask for a System Navigator.
