When a single parent’s car broke down, they were worried about being able to get to their two jobs and support their family.

In a state of distress, they reached out to our System Navigation team for help.
Our System Navigator quickly connected them with a local community service organization that sent a mobile mechanic to their home, completed the repairs, and even covered the cost.
Because of that fast action and strong community partnership, this parent was able to safely get back on the road and continue providing for their family.

Congratulations to Promise Community Health Center on their national recognition!

Their Hypertension Control Program was featured in Rural Health Models and Innovations for its innovative impact on heart health in rural Iowa. Impressively, the program’s blood pressure control rate increased from 73% in 2022 to 84% in 2024.

Kudos to the Promise team on this outstanding achievement!

We are incredibly proud to share that our Hypertension Control Program has been featured in the Rural Health Models & Innovations collection by the Rural Health Information Hub! This national recognition highlights the innovative work our team is doing right here in Sioux Center to improve heart health outcomes across northwest Iowa.

We know that rural healthcare comes with unique challenges like distance and financial barriers. That’s why we offer solutions like our transportation program, free follow-up visits, and loaner blood pressure cuffs to make sure every patient can access care. By focusing on these barriers and implementing a team-based approach, including nurse health coaching, remote monitoring, and bilingual services, we’ve seen our blood pressure control rate jump from 𝟳𝟯% 𝗶𝗻 𝟮𝟬𝟮𝟮 𝘁𝗼 𝟴𝟰% 𝗶𝗻 𝟮𝟬𝟮𝟰! 📈👏

Thank you, River Hills Community Health Center, for bringing together team members for the Iowa Department of Health and Human Services, Medicaid leaders, and team members from the Iowa PCA for a discussion on the impact of area clinic closures. Conversations like these are important to ensuring community members can continue to receive the care they need.

Last week, River Hills welcomed state HHS and Medicaid leaders, along with the Iowa PCA, to discuss the impact of recent clinic closures in Ottumwa. Our CEO and team shared how these changes affect patients in Southeast Iowa and emphasized the need for continued investment to keep care local and accessible. We’re grateful for the chance to advocate for our communities and highlight the vital role community health centers play across Iowa.

In one case, a System Navigator worked for weeks to find housing for a family of six and their dog. Thanks to the System Navigator’s persistence, the family was able to secure space at a shelter, giving the family three months to save up and plan for longer-term housing.

Finding the right support can be overwhelming—but System Navigators are here to help.

An Iowan struggling with financial challenges and the risk of displacement from their apartment had reached out to all their connections without success. A System Navigator connected the individual with a local church willing to cover the deposit and backpay needed to secure housing. Through this connection, the individual found more than just financial relief; they were greeted with compassion and emotional support during a time when they felt alone.

The connection became transformative. The individual later joined the congregation and now serves as a volunteer, committed to giving back after their own experience. This story showcases the ripple effect of meaningful community support.

The Iowa Primary Care Association (Iowa PCA) has provided technical assistance and training to Iowa’s community health centers to support their ongoing commitment to provide quality, affordable primary and preventive health care to meet community needs since 1988.

The Iowa PCA is a non-profit membership association comprised of 13 community health centers and one migrant health program and is recognized as the trusted voice on healthcare, particularly concerning vulnerable populations. These organizations collectively serve more than 226,000 Iowans each year through over 800,000 patient visits.

Our work focuses on our mission of enhancing community health centers’ capacity to care with a vision of equity for all.
