Today’s Diabetes Awareness Month spotlight features Community Health of South Florida, Inc. (CHI), whose standardized and coordinated approach is improving diabetes care across their centers.

CHI is leading the way through:
- Provider education, a unified diabetes management algorithm, and standing orders for point-of-care A1c testing.
- Dedicated diabetes clinics, CGM availability, and a drug assistance program that ensures patients receive necessary medications.
- Strong community partnerships like Blue Circle Health and close follow-up for uncontrolled diabetes until patients reach their targets.

CHI’s model ensures that every patient receives consistent, high-quality care no matter where they enter the system.

HCN will be spotlighting health centers who are dedicated to improving diabetes outcomes during Diabetes Awareness Month. We’re starting off with Premier Community HealthCare Group, Inc., whose commitment to consistent monitoring and personalized support is making a meaningful difference for patients living with diabetes.

Premier’s approach includes:
- Healthy Living Coach opt-out program with personalized education and lifestyle guidance for patients with elevated A1c.
- Standardized A1c testing every 91 days supported by ongoing staff training to strengthen documentation and BMI interventions.
- Monthly provider report cards and quarterly quality challenges that drive continuous performance improvement.

Premier continues to strengthen patient care and plans to expand access for employees with dedicated Diabetes Group Class slots in 2026.

HCN welcomes Whiteside County Health Department to the ! They will be joining HCN to utilize our Epic Powered by HCN model. Not only does the Whiteside County Health Department (WCHD) deliver the traditional public health services like environmental, immunizations, infectious disease, and maternal/child health, but it also provides primary and preventative medical, behavioral health, dental, oral surgery, and optometry services through a federally funded community health center, The Whiteside County Community Health Clinic (WCCHC). We are excited to grow the and expand our reach within the Illinois community.
