Health Choice Network invites health centers and partners to submit a poster of success to present at this year's 32nd Annual Board Educational Conference. Submit a poster on research or quality improvement initiatives and/or innovations implemented with results that can be shared with other health centers.

All accepted posters will have their own space within the conference and you will have the opportunity to answer questions from conference attendees. Every February, we recognize American Heart Month as a way to spread awareness about the importance of cardiovascular health. According to the American Heart Association, heart disease and stroke claim more lives each year in the U.S. than all forms of cancer and Chronic Lower Respiratory Disease (CLRD) combined. HCN considers improving cardiovascular health a key health initiative, and is currently supporting efforts to improve patient outcomes in multiple ways.

HCN's Clinical Quality Team wore red last Friday for Wear Red Day to raise awareness for heart health.

The webinar, Improving Financial and Quality Performance in Value-Based Care & Medicare Shared Savings with eConsults, will explore how primary care practices and ACOs are using eConsults to improve quality, reduce unnecessary referrals, and lower total cost of care. We’re proud to be part of the conversation on how eConsults can strengthen your ACO performance.

Last Saturday, February 7th, was National Black HIV/AIDS Awareness Day (NBHAAD). At Health Choice Network, we stand united in the fight against HIV/AIDS, celebrating the progress made in reducing new diagnoses within the community. As we work to reduce the burden of HIV and other health risks, knowing your status is key. Speak with your healthcare provider to explore testing options, including the convenience of self-tests. Let’s keep pushing forward, together.

Health Choice Care (HCC), our Medicare Accountable Care Organization (ACO), is growing, and so are the possibilities. In 2026, HCC welcomed additional participating health centers, expanding to 28 centers across 9 states. Together, we are advancing community-driven, value-based care through stronger collaboration, greater impact, and shared success. By scaling best practices and strengthening primary care, we’re making value-based care work where it matters most.

We remember and honor the life and legacy of Seferino Montano, a servant leader whose faith, compassion, and vision shaped access to healthcare in New Mexico and beyond. In addition to his decades of leadership at La Casa Family Health Center, Seferino played a pivotal role in launching Health Choice Network of New Mexico, helping expand HCN services into the state and laying the groundwork for a model that would later support national network growth.

His leadership helped shape early health center–controlled networks and advanced the development of multi-state infrastructure to support health centers through shared systems, training, and collaboration. Seferino’s legacy lives on in the communities he served, the organizations he helped build, and the countless lives touched by his unwavering commitment to care for those most in need.

HCN brought our Boards of Directors together at the end of January for a focused Strategy Planning Session centered on what's next and what it will take to get there. We're grateful to our Board Members for their time, perspective, and leadership. Their feedback and guidance continue to strengthen our path forward and reinforce what makes HCN successful.

Join AllianceChicago, Health Choice Network and featured guests for our next Research Engagement Forum. The meeting, focused on Improving PrEP Uptake Among Community Health Center Patients, will be on January 20 from 3-4PM CT/4-5PM ET. The forum will spotlight preliminary findings from a study to improve the uptake of PrEP among Black women and the experiences of participating Community Health Centers.

January is Cervical Cancer Awareness Month, a time to raise awareness about cervical cancer and the HPV vaccine. Cervical cancer was once the leading cause of cancer death for women in the United States. Now, thanks to cervical cancer screening and the HPV vaccination, it is the most preventable of all female cancers. Women can lower their risk for cervical cancer by getting screened regularly and by getting vaccinated.

What an amazing and rewarding year! We wanted to take a moment to thank our HCN Family for all they have done for the community in 2025. Through our giving opportunities this year, we raised $37,525, sponsored over 185 children, and volunteered 288 cumulative hours. Thank you to our Associates, volunteers, partners, and friends of the network who made this possible. Because of your support, we were able to make a meaningful impact in the lives of those in need.
