Health Choice Network attended the 2026 CROI Conference in Denver, connecting with leaders, sharing insights, and advancing HIV research and care. During the conference, HCN’s Clinical Quality team presented their abstract, “A Community-Based Model for Enhanced HIV Screening and Timely Linkage to Care.” Having an abstract accepted at CROI reflects the impact of HCN’s FOCUS initiative and opens doors for collaboration and impact on improving outcomes for our communities. We’re proud to contribute to advancing HIV prevention and treatment!

Health Choice Network invites health centers and partners to submit a poster of success to present at this year's 32nd Annual Board Educational Conference. Submit a poster on research or quality improvement initiatives and/or innovations implemented with results that can be shared with other health centers.

All accepted posters will have their own space within the conference and you will have the opportunity to answer questions from conference attendees. Every February, we recognize American Heart Month as a way to spread awareness about the importance of cardiovascular health. According to the American Heart Association, heart disease and stroke claim more lives each year in the U.S. than all forms of cancer and Chronic Lower Respiratory Disease (CLRD) combined. HCN considers improving cardiovascular health a key health initiative, and is currently supporting efforts to improve patient outcomes in multiple ways.

HCN's Clinical Quality Team wore red last Friday for Wear Red Day to raise awareness for heart health.

At Health Choice Network, we stand united in the fight against HIV/AIDS, celebrating the progress made in reducing new diagnoses within the community. Health Choice Care (HCC), our Medicare Accountable Care Organization (ACO), is growing, and so are the possibilities. In 2026, HCC welcomed additional participating health centers, expanding to 28 centers across 9 states. Together, we are advancing community-driven, value-based care through stronger collaboration, greater impact, and shared success. By scaling best practices and strengthening primary care, we’re making value-based care work where it matters most.

We remember and honor the life and legacy of Seferino Montano, a servant leader whose faith, compassion, and vision shaped access to healthcare in New Mexico and beyond. In addition to his decades of leadership at La Casa Family Health Center, Seferino played a pivotal role in launching Health Choice Network of New Mexico, helping expand HCN services into the state and laying the groundwork for a model that would later support national network growth.

His leadership helped shape early health center–controlled networks and advanced the development of multi-state infrastructure to support health centers through shared systems, training, and collaboration. Seferino’s legacy lives on in the communities he served, the organizations he helped build, and the countless lives touched by his unwavering commitment to care for those most in need.
