While we already provide excellent care during office visits, Chronic Care Management (CCM) allows us to care for Medicare patients between visits. Medicare covers this program so we can spend time each month reviewing medications, labs, and care plans, coordinating with specialists, and checking in proactively — all with the goal of helping prevent ER visits and hospitalizations.
