Provider Departure Notice
Important Announcement Regarding Our Medical Team
Dear Patients,
We wish to inform you that Taylor Surin, PNP, will be leaving our medical practice effective 12/22/25. Over the years, Taylor has become an invaluable member of our practice family, caring deeply for patients and enriching the lives of everyone here with her dedication, expertise, and warmth.
We know how important your relationship with your provider is, and we are committed to making this transition as smooth as possible. For patients currently under Taylor's care, rest assured your medical records will remain secure and confidential. Our team is here to discuss your ongoing care and help you select another provider from our practice, should you wish to do so.

A Message of Gratitude
As many of you may know, my last day here at Wellstar Creekside Pediatrics will be December 22, 2025. As I prepare for this transition, I wanted to take a moment to share my heartfelt thanks.
It has been an incredible honor to take care of your children and to walk alongside your families over the past several years. Thank you for trusting me with your little ones during sick visits, well checks, milestones, and everything in between. Being invited into such an important part of your lives is something I have never taken lightly.
I will truly miss my patients… both those I had the privilege of seeing for every visit, and those I was fortunate to care for along the way. Caring for your families has touched me deeply, and the relationships built here (and memories made) will always hold a special place in my heart.
I am grateful for the families I was able to see in person to say goodbye, and I sincerely apologize to those I was not able to speak with directly before my departure. Please know that this goodbye is no less meaningful, and that you are also on my mind.
At this time, I am so excited for the opportunity to step into a new season that allows me to be home with my own children, and I leave with immense gratitude for the years spent caring for yours.
Thank you again for the trust, kindness, and support you have shown me. Your families will be truly missed!!
