🎄✨ The Holidays Are Here — And It’s a Good Time to Think About Health ✨🎄

As a vascular specialist, I spend a lot of time helping patients with artery and vein problems. But did you know that some conditions that you might not expect — like knee pain from osteoarthritis or uterine fibroids — actually have a vascular connection?

For example:
➡️ Knee osteoarthritis — inflammation in the joint can be driven by abnormal blood vessels. A procedure called Geniculate Artery Embolization (G*E) can target these vessels, reduce pain, and in many cases help people avoid knee replacement.

➡️ Uterine fibroids — these are benign tumors that survive and grow because of the blood vessels feeding them. Uterine Fibroid Embolization (UFE) blocks those vessels, shrinking fibroids and relieving symptoms — all without major surgery.

The best part? Both procedures are same-day, no incisions, and no rehab or physical therapy. That means patients can get back to enjoying what really matters — friends, family, and all the holiday fun.

It’s amazing to see how targeting the vascular side of things can really improve someone’s quality of life. Learn more at www.360vascular.com

🎁 Wishing everyone a healthy, happy holiday season!

For over 7 years, this woman has struggled with a painful, non-healing ulcer on her right leg. She saw multiple vascular specialists, underwent ablations and varicose vein removal — each time hoping for relief, only to watch her leg worsen again.

Not because of a lack of effort or compassion, but because the true diagnosis was never made.

A few months ago, an IR cautiously suggested she might have a rare condition called Klippel-Trénaunay Syndrome (KTS), though he had never actually seen a case. That uncertainty led her to us at 360 Vascular Institute, where my team and I were finally able to identify what had been missed for years.

KTS is a congenital vascular disorder involving:
- Capillary malformations such as extensive port-wine stains
- Abnormal veins and lymphatics
- Limb overgrowth and chronic swelling

Once you understand KTS, her entire leg makes sense — and so does the years of failed healing. Standard venous procedures simply cannot correct a complex vascular malformation.

For her, finally receiving the right diagnosis brought answers, validation, and hope after so much pain and frustration.

If someone has a large port-wine stain, chronic swelling, or wounds that refuse to heal, KTS must be considered. The correct diagnosis doesn’t just guide treatment — it can change a life.

🌟 Incredible 1-Month DVR Results I’m Excited to Share 🌟

One of the most frustrating things I hear from patients with Post-Thrombotic Syndrome is that they’ve been told “nothing can be done.”

In reality, for most people, that simply isn’t true.

I recently treated a patient in his 40s who had lived with 10+ years of swelling, ulcers, pain, heaviness, neuropathy, and skin discoloration from PTS. He underwent Deep Venous Reconstruction (DVR) with us — and his 1-month post-op progress has been remarkable:

✔️ Pain — gone
✔️ Swelling — 80% better
✔️ Heaviness/fatigue — 90% better
✔️ Neuropathy — gone
✔️ Ulcers — smaller, and several completely healed
✔️ Skin discoloration — significantly improving

His before-and-after photos at just one month tell the story better than I ever could.

For those unfamiliar, DVR is a minimally invasive procedure that restores proper deep-vein drainage, relieving the pressure and congestion that cause PTS symptoms.

If you’d like to learn more about DVR, here’s a blog post I wrote a while back:

Do your legs feel heavy by the end of the day?

These photos shows C4 chronic venous disease — a stage where poor vein function leads to leg swelling, skin discoloration, and varicose veins. Chronic Venous Insufficiency (CVI) occurs when the valves in the leg veins stop working properly, causing blood to go backwards down the legs towards the floor instead of back to the heart.

Common symptoms include:
• Leg swelling
• Skin darkening or thickening
• Aching, heaviness, or throbbing
• Visible varicose veins (not always present)
• Restless legs and cramps
• Neuropathy
• Feeling unsteady on your feet

CVI is very treatable, especially when caught early. At 360 Vascular Institute in Dublin, OH, we focus on minimally invasive treatments that address the root cause and help restore comfort and circulation.

I recently had the chance to reconnect with Dr. Scott Trerotola -- once my partner and now a lifelong mentor and friend. He is the Chief of IR at Penn Medicine and a true legend in the field of Vascular & Interventional Radiology.

Dr. Trerotola was in Columbus giving an excellent talk on IVC filters. His leadership and passion for advancing our field continue to inspire me.

I spent over a decade at the University of Pennsylvania as an attending, where I grew tremendously as a physician, honed my skills, and had the privilege of teaching and mentoring future generations of interventional radiologists. Those years played a major role in shaping my career, and I will always value the time that I spent there.

I often meet patients who have spent months at a wound care center — and their ulcer still isn’t healing.

Most of the time, it’s not because of the wound itself, but because of circulation problems that haven’t been identified or treated. Healing takes teamwork: podiatrists, wound care clinicians, and vascular specialists working together toward the same goal.

In my latest article, I talk about what can go wrong when that connection is missing and how we can do better for our patients.

💡 How the Physician-Patient Partnership Drives Better Vascular Outcomes

I see patients from around the globe every day at 360 Vascular Institute, and what I’ve learned is that the most important part of care isn’t just treatment — it’s partnership. Whether someone is coming to me for peripheral arterial disease (PAD), vein disease, chronic pelvic pain, fibroids, or DVT, working closely with each patient, providing guidance, support, and encouragement, is what leads to the best outcomes.

In my latest blog post, “Under Pressure: How Weight Affects Your Pelvis and Legs,” I discuss how excess weight and sedentary habits can impact vascular and pelvic health — and how small, consistent lifestyle changes, combined with a strong physician-patient partnership, can make a meaningful difference in circulation, comfort, and overall well-being.

In good health,
Dr. Sudi

The Best Procedure Is Sometimes No Procedure At All

A few months ago, we saw a patient with PAD (poor circulation in both legs) who had been told that bypass surgery was his only option. No one had discussed medication, endovascular options, or even quitting smoking.

We took a different approach — focusing on education, empowerment, and conservative therapy. He started cilostazol (a medication that causes the arteries to dilate to improve blood flow), began a walking program, and worked hard to cut back from a pack a day to just five ci******es daily.

Now, he’s walking pain-free and motivated to quit completely. No surgery. No procedure. Just progress through commitment and the right plan.

We’re realistic. He may still need an intervention in the future, but only when he’s ready and after maximizing conservative therapy.

At 360 Vascular Institute, we believe great vascular care starts with patient-centered education and knowing when not to operate… and when to do so responsibly.

Teaching has always been more than a role for me — it’s been a calling.

For over a decade, I had the privilege of training countless residents and fellows at the Hospital of the University of Pennsylvania. That experience shaped not only the physicians I worked with, but also the physician I’ve become.

Even now, I continue to teach Penn Medicine residents through virtual lectures while building something new here in Dublin, OH — my own practice, 360 Vascular Institute.

What’s been most rewarding lately is mentoring the next generation of doctors through a new partnership with the Ohio University Heritage College of Osteopathic Medicine. These medical students are stepping into the clinic with fresh eyes, eager to learn about vascular disease and the future of minimally invasive care. But more importantly, they’re discovering the power of listening, connection, and trust which is the foundation of every strong physician-patient relationship.

Education doesn’t stop at graduation. And it doesn’t belong only in academic halls. Often, the most impactful lessons happen in the exam room, during a procedure, or while simply taking the time to explain something that truly matters.

This is how we shape the future of medicine.

🚨 PAD and CVI: The Overlap That’s Costing Patients Their Legs

Most people — and even many vascular specialists—don’t realize that arterial and venous disease often occur together. I've seen it every day in my career, and it’s the reason I coined the term VAM Disease (Venous and Arterial Mixed Disease).

The problem? Venous disease (CVI) gets ignored. It's not flashy. It’s often misunderstood. But it causes real harm—leg swelling, wounds that won’t heal, and pain that’s written off as “just aging.”

At 360 Vascular Institute, we take a truly comprehensive approach to vascular care:
🔎 We look for both PAD and CVI — because we treat both
🩻 We use advanced diagnostics to uncover what others miss
💉 We offer outpatient, image-guided treatments that change lives

This week, I had the chance to explain VAM Disease on NBC4's Daytime Columbus. I hope it starts a bigger conversation in vascular medicine.
