A common criticism of keto diets is that you only lose water weight. Considering that many people lose over a hundred pounds (more than 45 kg) on keto, that would be an awful lot of water! 💧

The truth is, you do release a lot of excess fluid when you cut carbs—in fact, this often happens within just a few days. But after that initial water loss, your body taps into stored fat and starts using it for fuel, leading to real fat loss, not just “weight loss,” and certainly not only water weight. 🔥💪

Plus, losing excess fluid is something you want. If your face or lower legs look puffy, or your fingers are swollen to the point that rings don’t fit, ditching carbs can help your body let go of that extra water. You’ll often see and feel changes before the scale catches up. (A super common non-scale victory: wedding rings fit again! 💍❣️)

Releasing excess fluid can also help improve hypertension, heart failure, edema, and other conditions tied to fluid retention. So yes — cutting carbs helps your body shed water… but that’s actually a very good thing! 💜✨

Scales aren’t the best way to assess how you’re doing on a keto diet — even when your main goal is weight loss. It’s not unusual for your size and shape to change even when your weight doesn’t move. (This is called body recomposition.) Plus, it’s totally normal for there to be weeks — even months — when the scale doesn’t budge. This doesn’t mean you’re doing anything wrong or that keto isn’t working anymore. It’s just human physiology! ⚖️💜

Staying at the same weight for a while doesn’t automatically mean you need to switch things up. But if it’s been a few months and neither your weight nor your size has changed, it might be time to take a closer look at your food or some lifestyle factors that could be slowing things down. 🔍

Dr. Eric C. Westman is an Associate Professor of Medicine at Duke University, and Director of the Duke Lifestyle Medicine Clinic.

Eric Westman combines clinical research and clinical care regarding lifestyle treatments for obesity, diabetes, and to***co dependence. He is internationally known for his work on low-carbohydrate nutrition. He is currently the President-Elect of the American Society of Bariatric Physicians, and a Fellow of The Obesity Society and the Society of General Internal Medicine. He has over 75 peer-reviewed publications in the fields of obesity, low-carbohydrate nutrition, and to***co dependence. He is on the Faculty of the Duke Clinical Research Training Program, and is the Course Director for a Fourth Year Medical Student Elective in Obesity Management. He has experience as an Associate in Health Services Research at the Durham Veterans Affairs Medical Center, a staff physician at the Duke Diet and Fitness Center Physician, and as an Ambulatory Care Staff Physician at the Durham VAMC, co-director of the Durham VA Stop Smoking Clinic, Director of Occupational Health at the Durham VAMC, and Resident Ambulatory Care Coordinator. Along with Stephen Phinney and Jeff Volek, he is a co-author of "The New Atkins for a New You" published in March 2010.

Dr. Westman was recipient of the Society of General Internal Medicine Associate Member Award for Outstanding Scientific Presentation and Internal Medicine Resident Teaching Award. He is a former Fellow of the North Carolina Governor's Institute on Alcohol and Substance Abuse. He received the ASBP Bariatrician of the Year award in 2010.
