Dear Valued Patients,

It is with heavy hearts that we share the passing of Dr.Anthony DiCostanzo.(Dr. D)

For over 40 years, Doctor D dedicated his life to caring for his patients and building lasting relationships within the community. He proudly practiced in Munster for 22 years before opening Smiles by Doctor D in Dyer in 2008, where he continued to provide compassionate, personalized care to so many.

Doctor D truly loved being a dentist—not just for the work itself, but for the people. His patients were an extension of his family, and the relationships he built over the years meant the world to him.

Due to his passing, it is with deep sadness that we announce the permanent closing of Smiles by Doctor D. This was not an easy decision, but one made with great care and consideration.

We want to express our heartfelt gratitude for the trust and loyalty you placed in him throughout the years. It was an honor to care for you and your families, and we are incredibly thankful to have been part of your lives.
