When patients face the obstacles presented by a critical illness, respiratory therapists play an essential role in restoring comfort and quality of life. Their collaboration with other rehabilitation specialists ensures patients receive the most comprehensive care possible.

“Every injury is unique and can involve significant challenges,” says Lori Vessell, respiratory manager at Select Specialty Hospital – St. Louis. “As respiratory therapists, we work closely with physical and occupational therapists and speech-language pathologists to develop a well-rounded approach to each patient. We mobilize patients being supported by the ventilator to prevent wound development and can give them back their voice by attaching a speaking valve, alleviating fear and anxiety.”

Our pharmacists and pharmacy technicians strive to administer the lowest effective dose of pain medication while ensuring that patients can participate in necessary activities. They coordinate these efforts alongside our multidisciplinary team as they discuss the needs of patients.

Michael Scott has a lot of faith in physical therapists. And he has a lot of faith in himself. When a motorcycle crash paralyzed his legs, it didn’t take long for the 29-year-old to move from acceptance to adapting to and overcoming his spinal cord injury. A physician-led team of physical, occupational and recreational therapists from HonorHealth Rehabilitation Hospital were there to help, with innovative technology and helping him develop strategies for everyday tasks like dressing himself.

Spinal cord injuries (SCI) demand highly specialized care. At Select Medical specialty hospitals, an interdisciplinary team of physicians, nurses, therapists, and dietitians and others collaborate to address the complex challenges patients’ face – from blood pressure fluctuations and pain, to muscle loss, pressure sores and digestive issues that can lead to malnutrition.

“Good nutrition plays a supportive role in recovery,” said Camilla Johnson, clinical dietitian, Baylor Scott & White Institute for Rehabilitation. She supports and educates patients with SCI during their rehabilitation. “In the hospital, good nutrition reduces the length of stay and … it enhances overall outcomes,” she said.

Laz Reeder checked into a hospital for hernia surgery, but aspirated during anesthesia and went into respiratory failure. The 35-year-old Florida man needed two life support machines to survive. A month later when Laz had stabilized, his wife Sierra turned to the continuum of care at Select Specialty Hospital - Pensacola and Select Medical Rehabilitation - Pensacola to continue his recovery and rehabilitation. Jim Cantu doesn’t remember passing out at work; his coworkers called 911. Turns out, Jim had a tear in his aorta. After undergoing surgery to repair it, Jim had setbacks that left him on a ventilator and unable to walk, talk or eat. He transferred to Regency Hospital – Northwest Indiana, a critical illness recovery hospital, and began the hard work toward rehabilitation. By the time he discharged only 16 days later, under the guidance of a physician-led, interdisciplinary team, Jim was walking, talking and eating – and ready for the next step in his recovery.

Don, previously an active retiree, was now on a ventilator following a heart procedure. He also couldn’t eat, walk or talk. A physician-led team that included nurses and therapists helped Don regain strength, mobility and independence to get back home with his family.

After a fall in his home left him unable to feel his legs, Terry Marcus underwent surgery to widen his spinal column and stabilize his spine. That’s when doctors found a spinal abscess, which was putting pressure on his spinal cord. Terry’s doctors told him he would be paralyzed from the waist down. Unhappy with that prognosis, Terry transferred to Regency Hospital – Northwest Indiana. The team at Regency Hospital developed a treatment plan aimed at helping Terry reach his ultimate goal—walking again.

Excruciating pain in his side led Juan Montalvo to seek emergency medical care where doctors diagnosed Juan with a severely infected abscess. Four weeks after the abscess was removed, Juan was transferred to Regency Hospital - Northwest Indiana with the goal of walking again and returning home. The interdisciplinary team at Regency Hospital implemented a personalized treatment plan that would help Juan on the way to achieving his goals.

Pulmonary rehabilitation—restoring a patient’s ability to breathe on their own—is a crucial aspect of the rehabilitation process in our critical illness recovery hospitals. Select Medical takes a team approach to pulmonary rehab, with every member of the interdisciplinary team playing a key part in helping a patient regain their ability to breathe independently.
Pulmonary Rehabilitation Week serves as a reminder of the critical role that respiratory therapy plays in advancing the healing journey. This week, we’ll show you how our physician-led interdisciplinary teams work together, implementing different therapies to help our patients breathe freely again.

Respiratory therapists use flutter valves (a tube that only allows air to move in one direction) and positive expiratory pressure therapy, which helps get air into the lungs so that it can be expelled forcefully enough to dislodge mucus, to help a patient clear their airway and strengthen the muscles needed for breathing. Timed breathing trials are performed to strengthen the lungs. During these short bursts, the patient spends increasingly longer stretches of time off the ventilator and without supplemental oxygen support.

Speech-language pathologists use a Passy-Muir valve – a specialized valve that allows mechanically ventilated patients to speak. It is placed on the end of a patient’s trach tube, not only to help them regain their voice but to strengthen their lungs. Swallowing exercises and a flexible endoscopic evaluation of swallowing (FEES) study can lead to resuming a more normal diet. Patients who can communicate and eat on their own are less anxious and more likely to participate in therapy.

Physical therapists implement a mobility protocol soon after the patient enters one of our critical illness recovery hospitals. By ensuring that the patient is sitting up twice a day and exercising their body, they grow stronger, improving their ability to breathe on their own.

Registered dietitians ensure ventilator patients are well nourished so they have the energy to participate in pulmonary rehabilitation. By adjusting a patient’s calorie intake to meet their needs, dietitians can slow weight loss and preserve the muscle necessary for patients to breathe independently.

Occupational therapists adapt activities of daily living to make it easier for patients to complete these tasks based on their current abilities. Common day-to-day activities, such as getting dressed, doing laundry, cooking and transferring between surfaces can seem impossible for pulmonary rehabilitation patients who have reduced endurance and strength. By teaching these patient’s adaptive strategies that reduce the energy needed to do these tasks, like sitting down to put your pants on or using a grabbing claw to reach far away objects, occupational therapists help their patients regain independence, conserve energy and build endurance over time.

Nurses are the backbone of every therapy team. They are the gatekeepers of care and a source of information for patients and their families. By monitoring and administering medications, they can help a patient achieve a state of wakefulness or painlessness that can make the difference during a day of therapy.

It’s World Delirium Awareness Day. Delirium is a condition experienced by up to a third of all people admitted for inpatient care. Its effects are wide-ranging and can be frightening. Patients may not recognize their loved ones or caregivers, or they may hallucinate. No matter the symptom, delirium is disruptive to the healing process and disturbing for patients and their loved ones.
