This patient has Fabry disease with apical LV hypertrophy and low native T1 values. Fabry disease is X-linked but female heterzygotes can be affected.

This patient was diagnosed with MINOCA involving the left circumflex (LCX) territory. Further evaluation revealed an anomalous origin of the LCX from the right coronary artery, confirmed by CT imaging.
