During Social Workers Month, we’re proud to recognize the incredible social workers across iCare Health Network who help guide residents and families through some of life’s most complex moments. 💙

Social workers play an essential role in skilled nursing and long-term care, supporting residents whose needs often extend far beyond clinical care alone. From leading care plan meetings and completing PASRRs to helping address behavioral health needs, cognitive challenges, housing insecurity, and discharge planning, their work is both deeply practical and profoundly compassionate.

They advocate, coordinate, problem-solve, and collaborate across disciplines to help ensure every resident receives thoughtful, individualized support — no matter how complex the situation may be.

Meet Sarah Krebs, Licensed Nursing Home Administrator at Touchpoints at Chestnut. Sarah chose long-term care because of her deep commitment to protecting vulnerable populations and safeguarding dignity when residents may be at their most dependent stages of life. For her, the complexity and constant challenges of long-term care aren’t something to avoid—they’re something to lean into. 💙

Sarah believes that “we all have the power to make a difference in someone’s life,” and she carries that mission into her work every day. Her leadership inspires her team to deliver care with compassion, ensuring residents and families feel supported during some of life’s most challenging moments.

For Sarah, the most meaningful part of her role is being a steady, strategic presence that keeps residents and staff safe and supported when it matters most.
