Last week, we proudly celebrated Activities Professionals Day, recognizing the dedicated Therapeutic Recreation, Life Enrichment, and Activities teams across iCare Health Network - the creative and compassionate professionals behind so many meaningful moments for our residents.

Quality of life is shaped by more than clinical care alone. It’s built through connection, engagement, laughter, and purpose. Our Activities Professionals bring these elements to life every day, designing programs that support physical, cognitive, social, emotional, and spiritual well-being - from group activities and celebrations to meaningful one-on-one interactions.

Established by the National Association of Activity Professionals (NAAP), Activities Professionals Day is observed annually on the fourth Friday of January to honor those who enhance quality of life through person-centered, therapeutic engagement.

Across iCare, our Activities Professionals foster community, champion holistic wellness, build trusted relationships, and create experiences that bring joy and meaning to daily life.

A great example of what coordinated, high-touch care can do. 💙

At Touchpoints at Chestnut, a resident enrolled in our Heart Failure Program and followed by the iCare Complex Care Team saw excellent results during their stay—including no edema, no 30-day re-hospitalizations, and strong improvements in key lab markers, supported by daily weights, weekly labs, and heart-healthy education.

Most importantly: the resident shared high satisfaction with the care and support they received.

Proud of this team—and the outcomes our residents deserve. 🙌

Touchpoints at Chestnut has been part of East Windsor’s story for more than half a century.

Long before today’s model of skilled nursing and short-term rehab, this Main Street building served older adults as D’Amore Rest Haven in the 1960s and 1970s—an era when care homes were smaller, more residential, and deeply connected to town life. As needs changed, the campus evolved too, becoming Chestnut Point Care Center and expanding clinical services while maintaining a close-knit, community feel.

In the 2010s, the facility entered its newest chapter as Touchpoints at Chestnut, joining iCare Health Network’s commitment to modernizing care while preserving the unique character of each campus. Today, we continue serving East Windsor as a 60-bed skilled nursing and rehabilitation center—still right here on Main Street, where our relationship with the community has always lived.

Here’s to a legacy of adapting, improving, and caring for neighbors—generation after generation.

Winter wellness is all about staying active, connected, and engaged in the community—and when we learned the East Windsor Senior Center has an active hiking group, it was an easy yes.

Last week, Touchpoints at Chestnut Administrator Sarah Krebs, LNHA and Transitional Care CHF Nurse Mirja Tupas, RN joined 20 local seniors for a three-mile walk at Dart Hill Linear Park.

This wasn’t a heart health presentation. It was walking side by side as neighbors—sharing stories, experience, and a little fresh air. Wellness in action. â„ïžđŸ„Ÿ"
