Duties
Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility, including the safe operation of Hoyer lifts and other assistive devices
Monitor and record vital signs accurately, including blood pressure, pulse, temperature, and respiration rates using EMR (Electronic Medical Record) systems like Cerner or eClinicalWorks
Support medication administration under supervision, ensuring proper documentation in medical records while adhering to HIPAA regulations for patient privacy
Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques and behavioral health support
Assist with complex clinical procedures such as catheter care, phlebotomy preparation, aseptic techniques, and infection control practices in accordance with hospital protocols
Collaborate with multidisciplinary teams in acute care settings such as Level I Trauma Centers or ICU units to deliver patient-centered care aligned with physiology knowledge and medical terminology standards
Support patients during transfers using Hoyer lifts or other mobility aids while maintaining safety protocols for both patient and caregiver
Experience
Valid certification as a CNA with current licensure in the state of practice
Home Care Experience
Prior experience working in diverse healthcare environments including hospitals), nursing homes, assisted living facilities, or home & community care settings
Demonstrated proficiency with EMR/EHR systems such as Cerner or eClinicalWorks for accurate medical documentation and patient records management
Experience providing care for individuals with disabilities or behavioral health needs; familiarity with dementia or memory care is highly valued
Strong understanding of HIPAA compliance and confidentiality standards to ensure patient privacy at all times
