Foundations Minnesota provides outpatient substance use and co-occurring mental health treatment for adults ages 18 and older at our two locations: Eden Prairie and Minneapolis Foundations Minnesota provides outpatient substance use and co-occurring mental health treatment for adults ages 18 and older at our two locations: Eden Prairie and Minneapolis. Our outpatient programs are designed to support individuals who want flexible care while continuing to live at home and maintain work or family responsibilities. Services include Partial Hospitalization (PHP) with optional housing, Intensive Outpatient (IOP), and Virtual Outpatient treatment, with individualized care plans based on a comprehensive clinical assessment. Foundations Minnesota at Minneapolis is the home of the PRIDE Program, providing LGBTQ+-centered outpatient addiction treatment and mental health services. Since 1986, PRIDE has specialized in culturally responsive care that addresses the unique experiences and stressors faced by LGBTQ+ communities, and provides a strong alumni community, helping clients stay connected to peer support and ongoing resources after treatment. As part of the Foundations Recovery Network of care, Foundations Minnesota connects individuals to appropriate levels of support while helping them take meaningful steps forward in recovery. Physicians are on the medical staff of Foundations Minnesota but, with limited exceptions, are independent practitioners who are not employees or agents of Foundations Minnesota. Ashley Collignon is a dedicated and culturally responsive addiction counseling professional with a strong foundation in behavioral health and holistic care. She studied Business at Ashford University and will complete her Bachelor’s degree in Addiction Counseling from Ottawa University in June 2026.

Ashley currently holds her ADC-T (Alcohol and Drug Counselor–Trainee) credential and plans to pursue her Master’s degree in the fall to become a Licensed Professional Clinical Counselor (LPCC).

Ashley began her work in the behavioral health field as a Behavioral Health Technician with the St. Croix Chippewa Tribe of Wisconsin, where she also completed a 16-week internship. During her time with the Tribe, she immersed herself in cultural trainings and hands-on learning to better serve her clients and community. Her experiences included becoming a certified butcher, learning traditional methods of collecting birch and maple sap and making syrup, ricing, and participating in sacred cultural outings. These experiences strengthened her understanding of culturally grounded healing practices and deepened her respect for community-centered wellness. Ashley later completed an additional internship at Sage Prairie in Eagan, where she further developed her clinical skills in both individual and group therapy settings.

Open-minded and compassionate, Ashley integrates a holistic approach into her work, drawing from her academic training, cultural experiences and personal travels. She believes in treating the whole person mind, body and spirit and is committed to creating a safe, inclusive environment where clients feel seen, heard and empowered in their recovery journey.

Employee Spotlight: Scott

We’re thrilled to highlight Scott, one of our amazing nurses here at Foundations Minnesota. Scott has been with us for about a year, bringing with him over 30 years of nursing experience and more than 25 years of sustained recovery—a combination that deeply enriches the care he provides.

Outside of work, Scott is an avid cyclist, a passionate oil painter and a talented violinist. When he’s not creating or riding, you can usually find him enjoying the peace of the outdoors at his cabin.

We are so grateful for Scott’s compassion, dedication and the steady presence he brings to our team. Thank you, Scott, for all the incredible work you do!
