Foundations Minnesota provides outpatient substance use and co-occurring mental health treatment for adults ages 18 and older at our two locations: Eden Prairie and Minneapolis Foundations Minnesota provides outpatient substance use and co-occurring mental health treatment for adults ages 18 and older at our two locations: Eden Prairie and Minneapolis. Our outpatient programs are designed to support individuals who want flexible care while continuing to live at home and maintain work or family responsibilities. Services include Partial Hospitalization (PHP) with optional housing, Intensive Outpatient (IOP), and Virtual Outpatient treatment, with individualized care plans based on a comprehensive clinical assessment. Foundations Minnesota at Minneapolis is the home of the PRIDE Program, providing LGBTQ+-centered outpatient addiction treatment and mental health services. Since 1986, PRIDE has specialized in culturally responsive care that addresses the unique experiences and stressors faced by LGBTQ+ communities, and provides a strong alumni community, helping clients stay connected to peer support and ongoing resources after treatment. As part of the Foundations Recovery Network of care, Foundations Minnesota connects individuals to appropriate levels of support while helping them take meaningful steps forward in recovery. Physicians are on the medical staff of Foundations Minnesota but, with limited exceptions, are independent practitioners who are not employees or agents of Foundations Minnesota. This will allow a person seeking the help they need to continue to work, go to school and live their life with minimal interruptions, all while getting back on your feet. With Foundations Minnesota offering both Partial Hospitalization Program (PHP) and Intensive Outpatient Program (IOP), we feel it is helpful to describe the difference.

Partial Hospitalization Program (PHP):
-Intensity: Higher level of care, often 5-7 hours per day, 5 days per week, providing a structured environment for acute symptoms.
-Focus: Stabilization, transitioning from inpatient care, preventing relapse, offering comprehensive therapy (individual, group, psychoeducation) and sometimes medical monitoring.
-Best For: Those needing significant structure and support but not 24/7 inpatient care, often after a residential stay.

Intensive Outpatient Program (IOP):
-Intensity: Less intensive than PHP, with fewer hours per day and week (e.g., 3-4 hours per day, 3-5 days per week), offering flexibility.
-Focus: Skill-building, maintaining recovery, integrating back into daily life while still needing structured support.
-Best For: Individuals stable enough to manage some daily responsibilities (work/school) but still need consistent, part-time support.

Think of PHP as a bridge from inpatient to outpatient, providing nearly full-day care, while IOP is a bridge from standard outpatient to independent living, offering focused, part-day support.

We want to give a huge welcome to our new counselor Kristine at our facility in Eden Prairie.

Kristine is excited to join the team at Pride in her role as a counselor. She is an ally to the LBGTQ+ community and aligns with the person-centered approach of care, values and authenticity that Pride provides. She is passionate about helping others on their recovery journeys and is looking forward to being able to support clients in their time at residential.

In her free time, she enjoys spending time in nature, anything fitness- specifically indoor cycle, painting, cooking and spending time with friends and family.
