Do you have a high school student with an intellectual or developmental disability who will need support after graduation?

The Somerset County Transition Resource Fair helps families understand what comes next after high school and how to plan for adult services with confidence. This event brings together local service providers and resources to support students transitioning into adulthood.

Families will learn how to access New Jersey Division of Developmental Disabilities (DDD) services, understand how Support Coordination works, and connect directly with Somerset County providers offering employment supports, community programs, and long-term services for adults with disabilities.

This fair is designed for families who want clear, practical information to help guide transition planning and decision-making after graduation.

ACI has teamed up with Saint Peter's University Hospital, Community Mobile Health Program to conduct free blood pressure and blood sugar screenings as part of our series of health and wellness workshops. Appointments are required. Fasting is not required for this screening.
