Chances are if you've tried and failed physical therapy, maybe had an epidural that didn't work, your Pain Management Doctor is now recommending a Medial Branch block in preparation for a Radiofrequency Ablation.

Stop and listen!

Burning those nerves will paralyze the Multifidus, the small muscle supporting each segment. Without this muscle it is likely you will further the instability that is already underway.

Instead, consider stimulating this nerve for equivalent, or better, pain relief. Add some PT back in once the pain is tolerable and you are now set up for long term success!

Advanced Practice Providers (APPs) are an integral part of the medical system as we are able to provide much (but not all) of the same services as a physician for a lower cost. So how do you know when it's right to add an APP to your team?

Generally it depends on two things.

1) You want to increase patient access to care

2) You want to increase revenue

The best opportunity occurs when both are true. But if the goal is simply increasing access, generally this will come at a cost to you. There may not be sufficient volume to make the position profitable but patient satisfaction may justify this cost in the long run.

If revenue is the goal, ideally the MD will already be nearing his max capacity. The new APP role can expand access while increasing MD capacity for higher complexity visits and interventional or surgical procedures. Once the APP has maxed out their capacity AND if the MD has room for more...then you rinse and repeat!

Now of course it's not as simple as plug and play, more on that another time. But here you have some framework on how to approach expanding!

Over exaggerating pain scores is often a manipulation by patients, but not necessarily for the reason you may assume. It often stems from a lack of confidence, not being taken seriously in the past, or feeling like they are not being heard. Of course there are always folks who catastrophize for an ulterior motive and clinicians need to be aware of this too.

Regardless, patients should understand that reporting realistic pain scores is much more likely to establish an empathetic provider. It's difficult to ascertain a reasonable treatment plan when you really don't know how much pain the patient is actually in.

The salary tool is missing an important component to contract negotiation. While it's great to compare salaries across specialties, there is so much variance in productivity that you may very well be cutting yourself short. The salary report does not take this into consideration.

Why should a PA bringing in $800k in revenue be paid the same as another PA bringing in $500k?

I'd argue knowing your revenue is a more valuable negotiation tool than the AAPA salary report.

.mcginn and I held our first APP Leaders in Pain meeting last night and it went better than expected! Attendees included 8 APPs covering Neurosurgery, Pain Management and Orthopedic specialties. We had some excellent engagement, discussion and interest throughout the evening.

Topics included:

1) Professional Growth inside and out of the clinic
2) Navigating insurance
3) Creating our network of local APPs
4) Leadership opportunities within industry, state and national organizations.

Thank you to for organizing another great Expanding APP Education course. We covered HFX neuromodulation, Adaptive AI and The Nevro1 SIJ fusion device. Events like this allow APPs to learn and stay up to date with data driven technology for our patients.
