Hospice provides support and care for individuals and their loved ones as they face the last phases of life – limiting illness. The goal of our hospice team is to make patients physically, emotionally and spiritually comfortable so they remain at home as long as possible with minimal disruption to normal activities. The hospice team can help with and offer nursing care, advanced nurse practitioners, medical equipment, home health aide services, medication symptom control and pain relief, short term hospital care as needed, social work support, spiritual needs and grief counseling 13 months after the date of death.

Happy Home Care & Hospice Month!
Today, we are recognizing our Illinois therapy teams!
Physical therapists provide home exercise programs to patients to assist with reducing pain and restore mobility, strength and balance.
Occupational therapists assist patients with overcoming or learning to live with disabilities by teaching them special techniques or the use of special equipment.
Speech therapists evaluate and develop a treatment plan to improve speech, language and swallowing.
HSHS Home Care offers all of these therapy services for all ages of patient from pediatric-geriatric. As therapy progresses, referrals may be made to outpatient therapy facilities to support patients in reaching their maximum potential.

Happy Home Care & Hospice Month! Our nursing staff provide skilled care to our patients across 37 counties!
As case managers, they communicate with the patient’s care team, including their physicians to ensure high quality health care and in the effort of preventing hospitalization. They provide wound care, IV therapy, cardiac rehab, fall risk assessments, pain management, pediatric care, and stroke care, along with so much more. They provide individualized care for each patient, including health education to the patient and family to promote continued independence and healing. Referrals to additional resources as needed are also made.

HSHS Home Care provides services to 37 counties throughout southern and central Illinois, along with 16 counties in eastern Wisconsin. We are a comprehensive home care agency that includes registered nurses, certified nursing assistants, licensed social workers, physical therapists, occupational therapists and speech language therapists. We are open 7 days a week with nurses on call throughout the night to assist with patients' needs. We provide services from birth to death. Specialty services include but not limited to advanced wound care, IV therapy, cardiac rehab, fall risk assessment, pain management, pediatric care and stroke care.
Home care provides care/education to the patients on service. Skilled nurses evaluate, monitor and teach patients about their medical conditions and assist with administering IV medications, tube feedings, wound care. Home health aides provide assistance with personal care and light housework within the homes. Physical therapists provide home exercise programs to patients to assist with reducing pain and restore mobility, strength and balance. Occupational therapists assist patients with overcoming or learning to live with disabilities by teaching them special techniques or the use of special equipment. Speech therapists evaluate and develop a treatment plan to improve speech, language and swallowing. Social workers provide counseling and referrals to the community agencies for caregivers, financial assistance and transportation.

Did you know?? In FY25, our Illinois home care colleagues drove 1,734,929 miles, Illinois hospice colleagues drove 429,728 miles, and our Wisconsin home care colleagues drove 495,833 miles! That is 2,660,490 miles (roughly 5.5 round trip visits to the moon!)!

Our team at HSHS Hospice Southern Illinois delivers our promise to provide high-quality Franciscan healthcare through two programs: Supportive Care and Hospice Care.
The goal of Supportive Care is to improve how a patient functions each day as well as improve his or her quality of life throughout the course of their life-limiting illness. Supportive Care addresses physical, emotional and spiritual concerns when patients and their families are coping with their illness. Timely attention to early management of symptoms has been shown to increase quality of life, improve function, and even extend ones length of life. Supportive Care can be delivered at any stage of illness beginning at the time of diagnosis. Supportive care can be provided while they are receiving curative or life-prolonging medical treatment. Such treatments may include chemotherapy, radiation, and surgery.
Hospice is a comprehensive program that focuses on quality of life. We recognize dying as part of the normal process of living. We affirm life and neither hasten nor postpone death. Hospice provides support and care for individuals and their loved ones as they face the last phases of a life‑limiting illness.
Our interdisciplinary hospice team includes advance nurse practitioners, registered nurses, certified nursing assistants, licensed social workers, chaplains, and volunteers.
We also offer the Everlasting Memories program to patients on our hospice service. This program allows us to grant wishes to our patients and provide them with an item or event to cherish in their final moments.
