In general, at 41 weeks with an unfavorable cervix, I would recommend a repeat Cesarean section. In cases of VBAC, the risks of a failed induction at 41 weeks with an unfavorable cervix is greater than 50%.

If a patient desires to wait for natural labor, I would recommend NST/BPP every 3 days until 42 weeks.

If the patient reaches 42 weeks, I would encourage her to consider her options for a Repeat Cesarean section vs Induction of Labor.

With the assistance of a VBAC advocate the assistance of a VBAC is 85% or higher even with a Previous Cesarean section x1 or x2 with an unfavorable cervix.

In order to improve the appearance of the abdomen after Cesarean section and reduce the risk of permanent abdominal muscle separation and "post baby belly" I recommend the reapproximation of the abdominal muscles during a Cesarean section. bones 🧟‍♀️

Don't ever call the Ex-wife or Ex-girlfriend assuming that you are talking to the pregnant patient. Depending on how much is shared during the call, it may not be a HIPAA violation vs a serious violation requiring an investigation. .issues

Under no circumstances should a patient be placed in a room with camera without expressed knowledge and written consent that their procedure will be recorded. Cameras can be accessed by staff directly within the office or off set remotely without the knowledge of the staff or patient.

In general, 25% of the US population carries the Herpes Simplex Virus, Type 2, aka ge***al herpes, without knowing it.

HSV Type 1, aka the cold sore, is prevalent in about 75% of the US population.

The clinical markers for exposure and subclinical infection for HSV 1 and 2 are Immunoglobulin M and G.

Past exposure is generally screened by IgG HSV 1 and 2.

Since HSV 1 and 2 are so prevalent, would you still like to know if screening is offered? Especially if you are pregnant???

As a standard of care, once you enter a Doctor's office, there is an immediate expectation of privacy protected by HIPAA. As such, recording is prohibited. Depending on the State, if you don't have Consent to record, you could face criminal prosecution and civil penalties.

Why am I a male OBGYN?

I am a male OBGYN. My father, now a retired OBGYN, was a mentor and one of my best teachers. Why OBGYN? There is not a single medical specialty that compares to the ability to help bring a new life into the world. When taking about going to the hospital, people get excited when it is talking about a baby being born. I give thanks to God that I have had the privilege of delivering over 10,000 babies including my three children. That same level of compassion and empathy extends to Gynecology. That is why there are male OBGYNs just like there are female OBGYNs. It is not about being gender restricted, it is about the appreciation of the life of female gender from birth.

As a caveat, male OBGYNs see transgender male and female patients so the specialty is not restricted to biological or cis female s*x patients. Kumin
