Doulas help provide a nurturing and supportive environment for a patient during labor and delivery especially considering that most providers are only present for a short period of the actual labor process. But there is a distinction between advocacy and informed consent. As your Doctor, we are also your advocate and our job is to properly inform you. When it becomes "necessary" for your doula to give you the medical information that we have not properly and clearly explained, then as your OBGYN, we have failed you.

The Webster Technique is promoted by chiropractors to help vert a breech presentation to a head down/ vertex presentation. There is even a certification program offered by chiropractic organizations for this technique. However, there are no peer reviewed studies supporting its efficacy especially considering the fact that by 37 weeks 94% of babies are in the vertex presentation. So, doing nothing and allowing the baby to turn on its own has better stats than the Webster Technique. barrington

VBAC calculators are no more accurate at determining the success of a TOLAC/VBAC than flipping a coin. The most important determining factor of TOLAC/VBAC is the experience of the provider.

Based on my 30 years of experience, the benefis of waiting when a patient is 36 weeks, 5cm, Gestational Diabetic and having "non-reactive" or "failed" NST DO NOT outweigh the risks. There are literally dozens of recent stories where patients go into labor or have spontaneous rupture of membranes and deliver at home, in their car, parking lot or ER because they progress from 5cm to full dilation precipitously. More concerning is the risk of cord prolapse which is a medical emergency wirh advanced dilation and non-engaged head or floating presentation. Johnson

Pitocin does not necessarily cause stronger or more frequent contractions. It is dose dependent. If you want it to match more closely to natural oxytocin, keep Pitocin under 10mu/min. Although Endorphins are released in response to oxytocin, they are also released during stress, anxiety, pain, anger and laughter. Even with painless contractions following an Epidural, endorphins can be released if you have a nurturing team to include the FOB, your family, doula, midwife, nurses and doctors. | Labor & Delivery RN

I still think of this baby all the time remembering back now 13 years ago.

This is one of my miracle babies. Born at 23 weeks after his mom had spontaneous rupture of membranes for almost 4 weeks.

We agreed to take it day by day rather than termination.

And he was born one of the smallest and cutest of the thousands that I have delivered.

Prays answered.

Points to Consider if a doctor claims to have performed a "Husband Stitch"

1. The Husband stitch is a cosmetic procedure involving the l***a, va**na and perineal body and one that is not an authorized or privileged procedure in hospitals.

2. Informed Consent must be signed in written form for a cosmetic procedure to be done.

3. Verbal consent alone is not enough to allow for this procedure to be done even when the patient consents.

4. Fewer than 5% of OBGYNs know how to properly perform this procedure.

5. The stitch is mistakenly done to make the l***a and posterior va**na smaller but this very often leads to l***al banding producing more pain doing in*******se.

6. Vaginal delivery is associated with permanent va**nal relaxation due to occult tears in the levator ani muscles, bulbocavernosus and perineal body, especially with babies born over 8lbs and multiple births over time. The Husband stitch cannot correct for this misconception.

The participation of OBGYNs in court proceedings to force women to undergo Cesarean section solely due to a previous history of Cesarean Section is a violation of our moral and ethical obligations to our patients. What type of Cesarean Section scar do you have? What is the benefit of the Micro-Brazilian over the Pfannenstiel and Joel-Cohen. When time permits, I make a Micro-Brazilian on the skin and a Joel Cohen incision through the fascia layers into the pelvis.
