Pitocin does not necessarily cause stronger or more frequent contractions. It is dose dependent. If you want it to match more closely to natural oxytocin, keep Pitocin under 10mu/min. Although Endorphins are released in response to oxytocin, they are also released during stress, anxiety, pain, anger and laughter. I still think of this baby all the time remembering back now 13 years ago.

This is one of my miracle babies. Born at 23 weeks after his mom had spontaneous rupture of membranes for almost 4 weeks.

We agreed to take it day by day rather than termination.

And he was born one of the smallest and cutest of the thousands that I have delivered.

Points to Consider if a doctor claims to have performed a "Husband Stitch"

1. The Husband stitch is a cosmetic procedure involving the l***a, va**na and perineal body and one that is not an authorized or privileged procedure in hospitals.

2. Informed Consent must be signed in written form for a cosmetic procedure to be done.

3. Verbal consent alone is not enough to allow for this procedure to be done even when the patient consents.

4. Fewer than 5% of OBGYNs know how to properly perform this procedure.

5. The stitch is mistakenly done to make the l***a and posterior va**na smaller but this very often leads to l***al banding producing more pain doing in*******se.

6. Vaginal delivery is associated with permanent va**nal relaxation due to occult tears in the levator ani muscles, bulbocavernosus and perineal body, especially with babies born over 8lbs and multiple births over time. The Husband stitch cannot correct for this misconception.

The participation of OBGYNs in court proceedings to force women to undergo Cesarean section solely due to a previous history of Cesarean Section is a violation of our moral and ethical obligations to our patients. What type of Cesarean Section scar do you have? What is the benefit of the Micro-Brazilian over the Pfannenstiel and Joel-Cohen. When time permits, I make a Micro-Brazilian on the skin and a Joel Cohen incision through the fascia layers into the pelvis.

Colposcopy done during pregnancy is generally not painful and performed in a way to avoid pain and bleeding. It will generally be repeated after delivery in the traditional way for comparison but may then be more uncomfortable.

I do not recommend attempting a VBAC at home or waiting at home while laboring in order for the hospital and/or doctors to support a VBAC attempt especially with a Previous Cesarean section x2 or more.

In general, at 41 weeks with an unfavorable cervix, I would recommend a repeat Cesarean section. In cases of VBAC, the risks of a failed induction at 41 weeks with an unfavorable cervix is greater than 50%.

If a patient desires to wait for natural labor, I would recommend NST/BPP every 3 days until 42 weeks.

If the patient reaches 42 weeks, I would encourage her to consider her options for a Repeat Cesarean section vs Induction of Labor.

With the assistance of a VBAC advocate the assistance of a VBAC is 85% or higher even with a Previous Cesarean section x1 or x2 with an unfavorable cervix.
