As students head back to school, it’s the perfect time to get clearer about their transition goals, their strengths, and the supports that help them thrive.

That’s why we’re sharing, Dude, Where’s My Transition Plan? , a booklet created for youth with disabilities, written in a voice that speaks directly to them.

Inside, you and your youth will find:

✔️ Transition planning worksheets

✔️ Easy-to-use checklists

✔️ Conversations about self‑advocacy and knowing your rights

✔️ Tools to help you shape the life you want after high school

Whether you’re just getting started or refining your next steps, this guide makes transition easier to understand and totally yours to own. 🌟

Before your child’s ARD meeting, prepare with intention. Ask for evaluations and draft IEP goals, review them ahead of time, and bring what truly reflects your child. Your voice matters—and being prepared helps you advocate with confidence.

Antes de la reunión ARD de tu hijo o hija, prepárate con intención. Pide las evaluaciones y las metas preliminares del IEP, revísalas con anticipación y lleva lo que realmente represente a tu hijo o hija. Tu voz importa, y estar preparado fortalece tu defensa.
