Early transfer of complex wound patients to settings with high-quality wound care can help reduce length of stay, readmissions, and care costs. Kindred Hospitals, which provides specialized acute care for medically complex patients, offers an advanced wound care program built on patient-centeredness, formal wound care training, team-based treatment, and standard protocols. These characteristics make Kindred a high-quality care setting for complex wound patients.

Beth was admitted to a local hospital in El Paso, Texas, after experiencing a severe illness and being diagnosed with pneumonia and sepsis. While at the local hospital, her condition worsened, leading to acute respiratory failure that required her to be placed on a ventilator. Once her condition stabilized, and after spending several weeks at the local hospital, a medical decision was made to transfer Beth to Kindred Hospital El Paso for advanced medical care, including ventilator weaning and extensive rehabilitation.

Prior to her admission, the care team meticulously reviewed Beth's medical history and condition, crafting a personalized care plan with a primary goal of ventilator weaning to drastically improve her recovery. The overarching objective was to help Beth regain independence and return home. Upon her arrival at Kindred Hospital, the interdisciplinary team swiftly initiated her care plan. Beth embraced the plan wholeheartedly with the support of her family and caregivers.

During her admission, Beth initially required maximum assistance to get out of bed and struggled with general activities of daily living (ADLs). Ambulating even short distances was challenging. Under the care of her pulmonologists and respiratory therapy team, Beth underwent extensive breathing exercises to strengthen her lung capacity and function. After dedicated efforts and close monitoring, Beth successfully weaned off the ventilator, a significant milestone in her recovery.

Following her liberation from the ventilator, Beth’s motivation soared. She eagerly participated in physical and occupational therapy to regain strength and mobility. Her rehabilitation included tailored exercises to rebuild muscular endurance and improve her functional abilities. With the guidance of her therapists, Beth utilized medical exercise equipment such as resistance bands, stationary bikes, and parallel bars to enhance her strength and coordination. She also engaged in balance training and functional movement exercises designed to simulate daily tasks. This comprehensive approach allowed Beth to progress steadily, improving her confidence and independence.

With the assistance of her therapists, Beth progressed to walking over 50 feet without supplemental oxygen and performing ADLs with greater independence. By the time of her discharge, Beth had achieved all the goals outlined in her care plan. Her remarkable progress allowed her to transition to a lower level of care for additional rehabilitation. From the entire medical staff at Kindred, we wish Beth all the success in her recovery.
