Home Ties Child Care and Development Center: Monday - Friday, 6:45AM - 5:30PM

Strengthening Families Resource Center: 8:30AM - 5:00PM

Closed: New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Christmas Day, Forest Festival Friday, and Black Friday.

• contact the Family Resource Coordinator (FRC) at 304-636-2363

**You must schedule a time with the FRC office (304-636-2363) to turn in your child's application and health forms for the process to be complete. **
Items you should bring to the meeting with the FRC:
• Basic Health History Form
• Completed Pre-K application
• Certified Birth Certificate from the State
• Well Child Exam- within last 12 months
• Immunization Record
• Dental Exam Verification Form- within 6 months
• Home Language Survey
• Legal/ Custody Information- if applicable
