MEDICAID SERVICE COORDINATION (MSC)-
Service coordinators assists individuals with intellectual and developmental disabilities and their families with gaining access to services, supports and entitlements appropriate to their needs. COMMUNITY HABILITATION-
Habilitation services assists the Individual by supplying the person with the means to develop maximum independence through support, training, education, etc. These services help the Individual to learn, keep or improve skills and functional abilities. Community Habilitation services provides the Individual with opportunities to learn and experience community based activities. Enhancement of aforementioned skills also occurs in the home. DAY HABILITATION WITHOUT WALLS-
Day Habilitation without Walls provides services in the community and is not based in a facility or “within walls”. The Individuals will generally meet in the community or are transported to places in the community and leave from a location in the community. A regular Day Habilitation program often provides services that are site based or in a facility and have times when a group will utilize the community. SUPPORTIVE EMPLOYMENT (SEMP)-
SEMP is Supported Employment, which is a program that provides on-going supports to Individuals in order to assist them with gaining and maintaining integrated, community based employment. The Individual is compensated at or above minimum wage and is working alongside workers without disabilities. BRIDGES TO HEALTH (B2H)-
This service provides skilled based activities to children who are in the Foster Care system.
