Honored to be back where it all began.
Last week I had the privilege of lecturing the UCSD Family Medicine residents and faculty on Transcranial Magnetic Stimulation (TMS) in Primary Care — focusing on the science, evidence, safety, and real-world integration into family medicine practice.

As a UCSD-trained family physician, standing in that room felt deeply full circle.
We talked about a reality that doesn’t get enough attention:
Family medicine sits on the front lines of mental health care.
Most antidepressants in the U.S. are prescribed by primary care clinicians.
Many patients never see psychiatry.
And after 1–2 adequate medication trials, remission rates drop significantly.

So what happens when:
• Medications partially work
• Side effects limit escalation
• Therapy access is delayed
• Patients remain functionally impaired

We need more than dose adjustments and referral lists.
We reviewed:
🧠 Depression as circuit dysfunction — not just serotonin
⚡How TMS works — targeted neuromodulation driving durable neuroplastic change.
📈 What the data show — meaningful response and remission rates, shorter-session theta burst options, and emerging accelerated models requiring careful interpretation.
📁 Safety and screening — bipolar rule-out, suicidality assessment, contraindications, and the essential role of primary care in patient selection.

The biggest takeaway?
Family physicians are not referral machines.

We are longitudinal brain doctors — whether we call it that or not.
If we prescribe the majority of antidepressants in this country, we should understand every evidence-based tool available to our patients.
Medicine is evolving faster than our training did.

And family medicine remains at the center of that evolution.

Grateful for the thoughtful questions.
Grateful for the intellectual rigor in that room.
Grateful to be part of redefining what comprehensive primary care can look like.
The future of family medicine is not smaller.
It’s broader.
It’s smarter.
And it’s brain-based.

Inflammation has become the wellness industry’s favorite villain.
