Reasons to Use Home Health
- Patients with a new medical diagnosis or medication that may still require education or instruction
- Post-hospitalized patients needing further evaluation, assessment and/or teaching
-Recent stroke patients requiring physical, speech or occupational therapy
- Patients having recent surgery resulting in the need for short term, intermittent skilled care
- Individuals with a history of frequent falls
- Individuals with a recent illness changing functional ability
- Patients having hip or knee surgery
- Home Health is less expensive and can often be just as effective as care in a hospital or a skilled nursing facility

Eligibility & Criteria
- Care ordered by a physician
- Patient has need for a skilled service (RN, ST, PT)
- Must have a face to face encounter with physician
- Meet Homebound status – leaves home infrequently or for a short duration and requires considerable and taxing effort. Going to church, doctor's appointment, and occasional outings permitted.
- Patients may leave home for medical necessity, religious purposes and occasional absences for nonmedical purpose. Rehabilitation

Our licensed nursing staff is on call for emergency 24-hours a day and available nights, weekends and holidays

- Chronic Obstructive Pulmonary Disease

Referrals
Anyone – Family, friends, doctors, clinics or hospitals can request Home Health Services. A nurse will do an assessment and develop a plan of care with your doctor.

At Greenwood County Hospital, our patients and families are at the heart of every decision we make. Beginning January 1, 2026, Greenwood County Hospital and all affiliated offices (Eureka Clinic, Howard Clinic, and Home Health) will no longer be in-network providers with the following Medicare Advantage plans: Humana, Aetna Medicare Advantage, and Wellcare by AllWell.

This was not an easy decision, and we understand it may cause inconvenience. Please know that we remain committed to providing excellent, compassionate care to every patient who walks through our doors and our team is here to answer questions and help you understand your options moving forward.

We will continue to be in-network providers with the following Medicare Advantage plans: United Healthcare Advantage plans and Blue Cross and Blue Shield of Kansas Medicare Advantage plans.

The Medicare open enrollment period is October 15 through December 7. During this time, you can: Review your current Medicare Advantage plan, compare with other available plans and decide if you want to switch to a plan accepted by Greenwood County Hospital, provider clinics and the home health department.

Effective immediately, and until further notice, persons and businesses receiving water through the City of Toronto's public water supply system are under a boil order. The boil order was prompted when testing revealed that chlorine levels in the water supply in Toronto did not meet the minimum threshold.

Boil Water Advisory Issued for the City of Toronto, Woodson County

TOPEKA –The Kansas Department of Health and Environment (KDHE) has issued a boil water advisory for the City of Toronto public water supply system located in Woodson County.

Customers should observe the following precautions until further notice:

Boil water for one minute prior to drinking or food preparation or use bottled water.
Dispose of ice cubes and do not use ice from a household automatic icemaker.
If your tap water appears dirty, flush the water lines by letting the water run until it clears.
Disinfect dishes and other food contact surfaces by immersion for at least one minute in clean tap water that contains one teaspoon of unscented household bleach per gallon of water.
Water used for bathing does not generally need to be boiled. Supervision of children is necessary while bathing so that water is not ingested. Persons with cuts or severe rashes may wish to consult their physicians.
The advisory took effect on August 20, 2025 and will remain in effect until the conditions that placed the system at risk of bacterial contamination are resolved. KDHE officials issued the advisory because the City cannot currently maintain the minimum required chlorine residual throughout the entire distribution system. Failure to maintain required chlorine residual levels may put the system at risk for bacterial contamination.

Regardless of whether the public water supplier or Kansas Department of Health and Environment (KDHE) announced a boil water advisory, only KDHE can issue the rescind order following testing at a certified laboratory.
