🌿What Is an Annual Wellness Visit (AWV)? 🌿

Most insurance plans — including Anthem Blue Cross Blue Shield — require patients to complete an Annual Wellness Visit each year. This visit helps you stay healthy by reviewing your medications, checking vital signs, updating preventive screenings, and completing a short Health Risk Assessment questionnaire and it is different than comprehensive annual preventive visit (CPE)

💡 At Allure Wellness Center, we make it simple!
We usually perform the **Annual Wellness Visit during your regular follow-up, so you don’t have to come in twice. You might see two-line items on your statement — one for your regular visit and one for the wellness portion — both following AMA and Medicare guidelines.

✨ Anthem often reduces your visit cost by half when the AWV is completed and may even reward you with a $100 gift card** for doing it! and give our office a higher quality star rating for completing it.
✨To ensure full transparency, every patient will now receive an acknowledgment form to sign whenever an AWV is completed. You’ll also receive a summary of your visit.

Personal Announcement (Leadership Transition) 🌿
After many rewarding years leading and growing my medical practice, I’ve decided it’s time for me to step down from my leadership and medical director role at Dr. Malek’s Office, effective November 1, 2025.
Our clinic will continue to thrive under its new name — Allure Wellness Center — with Dr. Michael Lindley among others stepping in as the new Medical Director, working alongside our amazing providers to carry forward the same mission of quality, compassionate care that our patients deserve.
This decision wasn’t easy, but it’s the right time for me to focus on the next chapter, both personally and professionally (I will be focusing on my specialty in addiction medicine. I’m deeply proud of what we’ve built and incredibly grateful for the trust, loyalty, and support of our patients, staff, and community over the years.
