Fall Health Fair
Where: Frontier Family Practice

When: Wednesday, September 24th, 2025
Time: 7 a.m. to 10 a.m.

We will be starting at 7 a.m. to allow you to come in for your lab draw before work. The Health fair starts at 7 a.m. and no appointments are necessary.

During the health fair you can have lab work done and grab granola bar to eat afterwards.

Please remember that some of the lab work does require you to be fasting. For those that are hungry we will have granola bars, and juice to eat/drink after you have had your blood drawn.

We will be doing the following lab draws:

CBC - $25 (regularly $43)
Comprehensive chemistry profile - $35 (regularly $53)
Lipid panel - $35 (regularly $66)
TSH - $40 (regularly $86)
Free T4 - $35 (regularly $47)
Chemistry profile + Lipids + TSH - $90 (regularly $208)
Vitamin D - $50 (regularly $150)
PSA - $40 (regularly $93)
Hemoglobin A1C - $35 (regularly $49)

We will give you a sheet of paper that you can turn into your insurance if you would like. (Please note that you cannot turn this in to Medicare.)

Just over 13 years ago Valerie and Carmen started Frontier Family Practice.

Today is Valerie’s last day here at the clinic. She will be missed by all, but we are excited for her new adventures of being a travel Nurse Practitioner.

Just a reminder that Frontier Family Practice closed at 12 pm for New Year’s Eve and will be closed on Wednesday January 1st.

Frontier Family Practice will be closed on Tuesday, December 24th thru Thursday, December 26th to allow staff time to spend with their families during this holiday season. We will be open on Friday December 27th until 1:30.

SPORTS PHYSICALS:

Frontier Family Practice would like to encourage parents to get their children scheduled for their sports physical prior to the end of July. We offer sports physicals for $30 or it can be included in your child’s annual well child exam. (Please note some insurance require to be a year and a day in between well child exams.)

Fairfield School sport's practices start on:
High School Volleyball: 8-16-24
High School Football: 8-16-24
Cross Country: ?
JH Football: 8-19-24
JH Girls Basketball: 8-19-24
JH Tennis: ?

We don’t know the Augusta or Simms School sport's practices start dates.

Just a heads up that Fairfield School is requiring that the Sports Physical Form and all other requirements be uploaded to Dragonfly prior to the first day of practice in order to participate in fall sports.

Update: Position has been filled as of 10-27-23. We will introduce our new staff member after she starts in mid-November.

Frontier Family Practice is taking applications for a full-time position with our clinic staff. Primary job responsibilities include, but not limited to, checking in patients, updating insurance and patient demographics, faxing referrals, entering lab orders, rooming patients as needed, drawing blood, processing lab specimens, answering phones, scheduling patients, and following up on referrals. Applicants should know how to efficiently use computers. Benefits include flexible schedule, 6 paid holidays per year, retirement plan after 6-month probationary period, day-time hours, half-day Fridays. Drop in to pick up an application or call us and we can email an application to you.
