Community Nurse is a nonprofit, independent home care agency providing visiting nurse, hospice, private care, and wellness services to southeastern Massachusetts since 1916. In alignment with our mission to bring compassionate healthcare services home, we provide a range of comprehensive health care and social services to meet patient needs through personalized care in the community and within the privacy of home. Importantly, Community Nurse offers all members of our community access to quality healthcare no matter the economic environment, amount of insurance reimbursement, or complexity of illness. We strive to exceed expectations and continue to develop innovative programs and services to meet community health needs. Community Nurse was named in 2019 as one of the top 100 home care agencies in the country by HomeCare Elite and received five stars (the highest ranking) from the CMS for patient outcomes.

During one of our Community Education Program sessions, our educator noticed that a participant's rollator walker, an essential tool for his independence, was no longer safe to use. While a replacement was needed, the cost made it out of reach.

Thanks to the generosity of our donors and the support of grants, we were able to step in and provide him with a new rollator, helping him move forward with confidence and peace of mind.

Moments like this are not why people come to our programs, but they are a reminder of why Community Nurse exists. Your support allows us to respond when unexpected needs arise, ensuring individuals can remain safe, independent, and supported in their own homes and communities.

We are incredibly proud to share that our very own Hospice Volunteer, Madeline Cooke, has been named the Patriots Difference Maker of the Year by the Kraft Family and the New England Patriots Foundation!

Madeline was honored on the field at Gillette Stadium during a special pregame ceremony, recognizing her extraordinary dedication to our hospice patients and their families. As part of this prestigious honor, Community Nurse received a $25,000 grant to support our mission of providing compassionate, patient-centered care in the home.

Madeline’s unwavering commitment exemplifies the true spirit of volunteerism. Inspired by her own experiences supporting loved ones at the end of life, she pursued a certification as an End-of-Life Doula. Through this work, Madeline sits vigil through the night, ensuring that no patient dies alone, offering profound gifts of presence, comfort, and peace during life’s most tender moments.

Madeline’s compassion, dedication, and unwavering commitment truly embody the spirit of volunteerism. We are deeply grateful to have her as part of the Community Nurse family.
