The Four Grades of Whiplash Associated Disorders

WAD I is characterized by neck pain and stiffness without any objective findings on physical examination. In other words, there is no loss of range of motion; no muscle spasm or guarding; no swelling, bruising, or deformity; no neurological deficit; and no imaging abnormalities. Approximately 15–25% of whiplash patients fall into this category.

In WAD II, neck symptoms are accompanied by physical examination findings such as decreased range of motion, localized tenderness in neck muscles, muscle spasm, and sometimes headache. However, there are no neurological deficits or abnormalities visible on diagnostic imaging. About two-thirds of whiplash patients are graded WAD II.

At the WAD III level, patients present with both musculoskeletal findings (as seen in WAD II) and neurological signs, which may include sensory loss (numbness or tingling), motor weakness (reduced strength in muscles supplied by affected cervical nerves), altered reflexes, or radiating arm pain. As with WAD I and II, the injury still involves soft tissues that typically do not appear on X-ray or advanced imaging. Approximately 5–10% of whiplash patients fall into this grade of WAD.

The classification of WAD IV is utilized when there is structural damage to the cervical spine that is present on diagnostic imaging and is usually associated with severe symptoms. Patients with WAD IV typically require emergency treatment to stabilize the spine. Fortunately, fewer than 1–2% of whiplash patients meet this criterion

The good news is that WAD I, II, and III typically respond well to a multimodal chiropractic approach aimed at reducing pain and restoring function as quickly as possible. Manual therapies may include gentle, low-velocity, low-amplitude techniques; thrust manipulation (high-velocity, low-amplitude); facet gliding and long-axis cervical traction.

Despite many attempts (and failures) at finding a single diagnostic test to solidify the diagnosis of vertigo, the good news is that chiropractic care helps a lot! There is strong evidence that spinal manipulation of the neck and midback helps by stimulating the mechanoreceptors located in the neck muscles and ligament insertions. More specifically, the muscle spindle cells located in the deep short neck muscles and the sensory fibers connecting the facet joint proprioceptors with the spinal cord that feeds information to and from the vestibular (balance) system and other parts of the brain all play important roles in helping to resolve vertigo in WAD patients.

In 2011, a group of Australian researchers uncovered 14 prior studies that reported improvement in dizziness using both unimodal (manual therapy only) or multimodal (more than one) therapy interventions. They cited improvements in postural stability, joint positioning, range of motion, muscle tenderness, neck pain, and vertebrobasilar artery blood flow velocity in their systematic review. A more recent 2020 systematic review that included 22 clinical trials identified manual therapy, vestibular rehab (eye exercises), Tai Chi, and canal repositioning exercises as effective for the management of cervicogenic vertigo. Doctors of chiropractic may incorporate these methods into a treatment plan for the CV patient as well as team up with allied healthcare providers when appropriate.

Here are three exercises for strengthening the lower trapezius muscles used in the study:

Modified Prone Cobra: Lay on your stomach with your arms to your side, palms facing up. Contract the lower trapezius muscles without engaging the upper trapezius muscles and raise your chest about four inches. Hold for ten seconds.
Wall Slides: Stand with your back pressed to the wall have your feet a couple inches away from the wall. Keep your arms bent at 90 degrees and pressed against the wall so the outer side of your upper and lower arms touch the wall, palms facing out. Keeping the arms and low back pressed to the wall, slowly slide your arms upward to full extension. Hold for ten seconds.

Prone Lower Trap: Start in the position as the first exercise but place your hands behind your head while squeezing the scapulae together (adduct) lifting the chest and elbows towards the ceiling for ten seconds.

The authors reported that although both groups improved with respect to pain, disability, and postural alignment, the trapezius group showed more significant improvement in disability and postural alignment. The researchers concluded that the addition of specific lower trapezius muscle strengthening yielded the best results and recommended its inclusion in UCS care.

Doctors of chiropractic also have the option to utilize spinal manipulation, which is a single high-velocity, low amplitude (HVLA) impulse that’s delivered to a spinal region to improve movement where it is lacking. In the end, the choice of treatment can depend on several factors, such as patient and provider preference, the presence of osteoporosis or hypermobility, safety concerns such as near or in the region of hardware, and patient comfort during the pre-manipulative set-up.

A systematic review published in 2020 that looked at data from 51 studies found moderate evidence for recommending non-thrust therapies such as neuromobilization and myofascial release versus other manual therapies for the post-lumbar fusion PSPS-2 patient. The review did not determine that either thrust or non-thrust therapies are more effective for other surgical interventions and recommended more research on the topic. Until then, the most appropriate approach used will depend on the patient’s unique case, their treatment preferences, and their chiropractor’s training and clinical experience.

One particular condition, lumbar spinal stenosis (LSS), is characterized by a narrowing of the holes that the spinal cord passes through, as well as the nerves that branch off from the spinal cord and travel to the head, arms, trunk, and legs. The terms “central” and “lateral” stenosis apply to the spinal cord space and the nerve root spaces, respectively. It’s important to note that the blood vessels that travel with these nervous system tissues, the neurovascular structures, can also become compressed. One study, which analyzed data from the Framingham Study cohort, found that nearly half (47.2%) of adults in their 60s have some degree of LSS.

In addition to symptoms associated with LLS like low back pain and stiffness, patients may also experience poor standing and walking tolerance, which is referred to as neurogenic claudication (NC). When NC is present, patients will also report that one or both legs feel heavy, tired, achy, crampy, numb, and/or weak. They may also experience impaired balance, which can be a real problem because falls can significantly affect a senior’s long-term health and ability to remain independent.

The good news is that the majority of these patients do quite well with non-surgical care such as chiropractic treatment. A 2022 systematic review and meta-analysis LLS with NC reported that the current research supports an initial multi-modal, non-pharmacological treatment approach that includes patient education, rehabilitative exercises, and manual therapies—all of which can be provided by a doctor of chiropractic.

Unfortunately, as we age and depending on our lifestyle choices, the structure of the cervical disks can weaken, the annulus can tear, and part of the nucleus can leak into or beyond the annulus. This may be referred to as disk herniation, protrusion, extrusion, or sequestration. In some cases, this can occur in such a way that the protruding nucleus does not cause pain, which is something that is commonly seen on MRI scans of asymptomatic individuals.

However, in other instances, the bulging disk may press against a nerve root, creating pain that shoots down the course of the nerve. In the case of the cervical spine, pain may follow down through the arm and into the hand. The outer edge of the annulus also has a nerve and blood supply, so injury to the annulus can cause localized pain. Additionally, these nerves and blood vessels can proliferate deeper into an injured disk, leading to pain in the immediate area.

Researchers have also observed that the force of the disk pushing into the endplates of the vertebral bodies both above and below can lead to a fracture known as Schmorl’s node. It’s long been thought that these are painless, but a 2021 study that included 582 participants identified an association between Schmorl’s nodes in the cervical spine and local, non-radiating neck pain.

Chiropractic care offers a conservative multi-modal treatment approach that has been found to be highly effective for managing patients with neck pain arising from multiple sources, including cervical disk injury.
